Should you require a copy of your child's medical records FAX a HIPPA compliant Records Request to (405) 337-9635. Provide a valid voice number or email address as contact information and someone will be in touch.

Please do not send Protected Health Information in an unsecured email.

Even though the office is closed, we will endeavor to meet all properly formulated records request as volunteer time permits.
