On busy clinic days, that decision shouldn’t slow your team down, or turn into a debate at the treatment table. Modality choice should serve the plan, not drive it.

A simple framework holds up under pressure:
Start by filtering for irritability and how targeted the presentation is.

Highly targeted, load-sensitive spot that reproduces the patient’s primary complaint? A short, targeted shockwave application with an immediate functional re-test often makes sense.

Broadly sensitive, irritable, or flared-up presentation? Laser can be the cleaner first entry - contact-free, quick to apply with presets, and easy to pair with graded loading in the same visit.

Reassess after 2 - 3 sessions. Switch the sequence if irritability drops. Simplify to one modality if that best supports the day’s objective.

The goal isn’t to prove which modality is “better.”
It’s to move the patient forward while keeping your workflow predictable.

And none of it works without consistent functional re-testing. Measure what changed. Document why you chose what you chose. Keep it repeatable.
