Provide assessment and rehabilitation services, including diabetic therapy, IV therapy, wound care, cardiac case management, care of patients with terminal illnesses and nurse staffing services. Health First Home Health, Inc skilled nurses can provide education (e.g. Diabetes) to the individual and their family, coordinate the treatment plan with other team members, provide care, and communicate with the patient’s physician skilled nursing

It is our intent to provide skilled nursing care, physical and occupational therapy, speech language pathology, medical social services, and home health aide services, as well as the necessary guidance, education and instruction to the patient and others concerned for their care.
The specific focus is to promote and maintain the physical, emotional, spiritual, and psychosocial health to our patients; thus providing a holistic and multi-disciplinary approach to health care in the home. The Agency strives to work effectively with other health care providers and community organizers to promote continuity of care.
We have a strong commitment to education and promoting an increased knowledge of home health care. We strive to educate the community, physicians, health care professionals, and paraprofessionals about home health care services and related technological advances.
