All services are provided by appointment only, Monday through Friday, excluding Friday evenings
and Saturdays. Body and Soul Therapeutic Massage is notnopen Sundays or traditionally recognized Holidays. All clients are required to complete a Client History Form before or at their first appointment. The therapist will review the information and discuss it with you, should you have any questions or concerns. If contraindications are present, written permission from your medical or chiropractic physician will be required for the specific service/s. There will be no charge for the Client History Review. Payment per visit is due in full when services have
been completed. CASH and personal or business
checks are accepted. Insurance forms are not
provided or completed at this time. If your insurance covers any of the services you are receiving, in whole or in part, it is your responsibility to check with your carrier to submit your claim for reimbursement. Specific Therapy Codes can be provided to you for this purpose. Please call to schedule an appointment at least 24
hours in advance. Remember that your appointment time has been reserved specifically for you, the Client! To cancel or to “no show” at the last minute does not allow enough time to schedule anyone else in that appointment slot. Emergencies aside, we strive to minimize last-minute cancellations and no-shows. Thank you for your co-operation and understanding! If you have scheduled an appointment for a specific service and wish to change to a different service or treatment, please call within 24 hours before your appointment to ensure your change/s have been accommodated. Habitual lateness affects the overall results of your session, the recovery time for me between sessions and an unrecoverable reduction in my compensation so please be on time.
