Big airway dataset, practical takeaways:
• 2,839 intubations with known operator experience (median 56 prior)
• 65.6% critical care, 26% emergency, remainder anesthesia/other
• Adjusted for BMI, APACHE II, setting, and device type
• First-pass success came in at 81.5%, consistent with reports when learners are involved

Same learning curve, different subgroups: ED trends higher than ICU early on, and video laryngoscopy outperforms direct. Most of the lift happens between 0 and 100 prior intubations, with a 35–50 “sweet spot” where skills consolidate, a small dip as confidence builds, and then gradual improvement.

Airway learning curve in one graphic:
• ~60% first-pass success at near-zero experience
• Rapid rise to 80%+ by ~20 tubes
• 35–50: brief leveling off and a small dip
• Then slight gains out to 300 prior intubations
Self-reported numbers, so keep that caveat in mind.

Learning curves come with mile markers. For airway, many learners hit a plateau around 35–50 intubations, which works as a practical competence threshold. It’s a guide—not a rule—so adjust to the person you’re teaching and build goals around it.

Oct 2024 Annals study: acute stroke admits, single urban center.
Outcome: total cost per patient for med-surg and ICU, borders vs inpatients.
Why stroke: early ED recognition, high admit rate.
Caveats: one ICU + one floor → consistent staff/overhead, easier buy-in; single-center limits generalizability.

What does care really cost? Not just the line item. It’s the time to perform each task and the dollars per minute for the people and space involved. In this article, theyteamed up with finance to pull salary and overhead data, then divided annual totals by exact working minutes for each role and care area. The result is a clearer per-minute rate and a more honest view of cost.

Pediatric DKA: keep cerebral edema on your radar.
Red flags: mental status changes, new or worsening headache, vomiting that returns after getting better.

In DKA, potassium levels can be misleading.
Potassium 3.5 with a low pH (like 6.8) can cause pseudo-elevation, meaning the true potassium level is likely much lower.
If severe acidosis and low-normal potassium, supplement with oral and IV potassium.
Watch this clip from Piece of Cake: DKA/HHS Update Part II with Dr. George Willis to learn how to safely manage potassium in DKA. Full episode and show notes on SimKit.

Use entitled CO2 to rule out DKA quickly in the ED.
Entitled CO2 > 27 → you can effectively rule out DKA
Entitled CO2 < 27 → doesn’t rule in DKA, but points to other metabolic causes

Learn how this tool helps narrow your diagnosis in this clip from Piece of Cake: DKA/HHS Update Part II with Dr. George Willis. Full episode and show notes on SimKit.

HHS isn’t a quick fix.

• Develops over time → needs a longer course
• Don’t chase “normal” glucose in the ED
• Slow correction to avoid sodium-related injury
