Even though we're mobile, you'll still have access to specialized equipment, like infrared video goggles, which are essential for the assessment and treatment of vestibular disorders and concussion. Vestibular Migraine and Ménière’s Disease have many overlapping symptoms, and diagnosis can be difficult.
Those with vestibular migraine are often mistakenly diagnosed with Ménière’s Disease.

Here’s a few differentiating factors:
- Ear symptoms in vestibular migraine is more often bilateral (both ears), and Ménière’s more often unilateral (one ear). Of course, it’s possible to be the opposite.
- Vestibular migraine can lead to fluctuating hearing loss/ muffled hearing as well. However, repeat hearing tests over years will likely show no change, whereas Ménière’s disease often leads to low frequency hearing loss over time.
- Photophobia and photophobia, as well as vertigo attacks lasting > 12 hours may occur more likely with vestibular migraine (however still possible with Ménière’s disease)
- Ménière’s disease usually presents as building ear pressure with difficulty hearing and a roaring tinnitus, followed by a vertigo attack. However, not everyone experiences all of these symptoms, especially early on in the diagnosis.

Migraine, and especially vestibular migraine, is a common cause of dizziness and vertigo.

Migraine is more than just a headache! And in some cases, you may not even experience head pain (like with vestibular migraine- headache is not always present)

Migraine is a complex neurological disorder and has multiple phases:
Prodrome, Aura, Migraine Attack, and Postdrome. The time in between is the interictal period. (Not everyone will experience all of these phases- and they may show up differently in each person).

The prodrome can occur a few hours to days before the next phase and may look like fatigue, cravings, mood and cognitive changes.

The Aura phase includes transient neurological symptoms (like odd visual changes) usually lasting minutes- an hour before a migraine attack.

The postdrome phase occurs after the attack, and can last up to a few days. It often feels like a hangover (feeling fatigued, irritable).

Groundings for Dizziness:
🌀 Grounding is a powerful tool for managing symptoms of dizziness.
🦶Grounding refers to increasing somatosensory input (the sensation coming from your body, skin, joints, etc) to improve body awareness. It can help your body better determine where it is in space and correctly orient itself when sensation from your vestibular or visual system are conflicting or inaccurate.
It can also help calm your autonomic nervous system.

Here are some examples:

- Sitting with feet flat and with back support. You may also try standing against a wall or laying down. You can couple this with your favorite breathing exercise, meditation, or music.
- Holding light weights, using a light weighted vest, or a weighted blanket.
- Explore different positions, for example, quadruped or child’s pose.
- Do a body scan with focus on all of your contact points.
- Axial loading: use your hands to provide a GENTLE downward pressure through the top of your head.

If you’re living with a vestibular disorder or post concussion syndrome- you’ve probably wondered- “how hard should I push it?”

The 2 point rule is a great guideline/ starting point for pacing an activity or exercise. (It’s not really a RULE, but more of a guideline).

The two is referring to a symptom scale that ranges from 0 to 10, with 0 being no symptoms and 10 being severe symptoms.

If you’ve noticed a +2/10 symptom increase- that’s a great time to pause, slow down, take a break, or use a symptom management tool or strategy.

This is one of my favorite grounding exercises, and you can do it just about anywhere and in any position!

- Get in a comfortable position, and shift your attention to each of your senses. Try to name 5 things you can see, 4 things you can feel, 3 things you can hear, 2 things you can smell, and 1 thing you can taste

Tips:
- When working on 4 things you can feel, try to notice things outside of your body. For example, you notice the chair pressing into your bottom, you fee the sun on your face, you feel the breeze on a fan blowing.
- If you find yourself to be very visually sensitive, you can skip the first part and focus on the other senses.
- If you just can’t come up with enough things (for example, there may not be 3 different smells available)- think of smells that you enjoy (you can do the same for any of the senses)

📱 screen time after concussion:
This recent study (“early post injury screen time and concussion recovery”) observed a “Goldilocks” 🐻 effect with screen time in the first 7-10 days after injury, where moderate screen time demonstrated less pronounced group differences (compared to orthopedic injury), than both high and low screen time.

This supports the recommendation of moderation of screen time, versus a blanket restriction, especially after the first 48 hours.

April is FND awareness month. FND stands for functional neurological disorder.
It’s a neurological disorder that can lead to various neurological symptoms like limb weakness, seizures, tremors, sensory changes, and dizziness- caused by a problem with the functioning of the nervous system and it’s ability to send and receive signals.

Functional dizziness is also called PPPD (persistent postural perceptual dizziness), which is a common and treatable form of chronic dizziness.

Older adults with BPPV (benign paroxysmal positional vertigo) often present with more vague symptoms of general dizziness and instability rather than classic symptoms of vertigo!
However, it’s not always tested in those complaining of dizziness, falls, or instability without classic symptoms..
Undiagnosed BPPV can lead to changes in gait, increased risk for falls and fractures, increased fear of falling, unsteadiness, increased depression, and reduced quality of life.

CC: Migraine is often misdiagnosed as a sinus headache or sinus issues. I’ve seen this happen many times!

Vestibular migraine is a type of migraine that causes vertigo and other vestibular symptoms.

Vestibular migraine can be present WITHOUT head pain.

PPPD stands for persistent postural perceptual dizziness. It’s a type of chronic functional dizziness.
One of my favorite mantras for those with PPPD is “sway is okay.”

CC: Understanding PPPD. Those with pppd may perceive that they are swaying more than they actually are, leading to increased perceived instability. This doesn’t mean that you’re making it up! There’s a “software” issue causing an error in magnitude perception.
This sensory error can be interpreted as a threat by your nervous system, leading to hyper vigilance and increase fight/flight response.. which can lead to avoidance and more sensitivity.

This is why I love the mantra “sway is okay.” - to help reduce the fight/ flight response that tends to get coupled with sway and movement.

Grounding exercises can also help to calm the nervous system and reduce some of the error signal.

Then you can incrementally reintroduce movement through graded return and habituation.
I recommend working with an experienced vestibular therapist to help you through this process.

This is a strategy that I use from my pulmonary rehab days, I’ve found it helpful as one way to incorporate both pacing and breath work in your daily activities.
It’s great if you also experience significant fatigue, if quick movements tend to increase symptoms, experience shortness of breath with activity, or if you have a pulmonary comorbidity.
Like most things, it takes practice and May or May not be the right strategy for you. Give it a try!
