Ever noticed the little note on your lab report that says “LDL-C (calculated)”? That’s a hint.

Most LDL numbers on a standard lipid panel are not directly measured—they’re calculated using formulas that assume your bloodwork was drawn fasting.

Here’s why that matters:

If you weren’t fasting, your triglycerides may be elevated from your last meal.

High triglycerides skew the LDL calculation, making it look worse (or better) than it truly is.

Decisions about diet, meds, or supplements could be based on bad math.

Bottom line: Fasting = clarity. Aim for 8–12 hours without food (water is fine). Then you can actually trust your numbers—and your plan.

You don’t need a PhD to make sense of a lipid panel—you need a process. Here’s the 3-step read I use to turn standard labs into a real plan.

Step 1 — Confirm fasting.
Most LDL values are calculated and assume fasting. If triglycerides are elevated because you ate, LDL-C can be off—leading to mixed messages.

Step 2 — Know your risk category.
Low, moderate, high, very high, or extreme? Your risk (age, BP, diabetes, kidney function, family history, inflammation, pregnancy history, etc.) determines how low your numbers should go.

Step 3 — Check the TG/HDL ratio.
A ratio ≥ 2.4 strongly suggests insulin resistance—a root cause we can treat with lifestyle, targeted supplements, and sometimes medications. Fixing metabolism often improves the entire lipid profile.
