I didn’t keep it a secret that I got all sorts of types of pajamas to get me through, but I didn’t realize I was gonna also break my back during the entire process as well.

Extremely overdue & very quick update for those who don’t know due to the extended SoMe break.

𝒮𝓊𝒸𝒸ℯ𝓈𝓈𝒻𝓊𝓁! / refashion Jan 29, 2025. ARFID is tough, and my current update makes it hard to attempt to push through.

March 2025, thoracic osteomyelitis seeded - Infectious Disease Dr (ID) treated it while not listening to [me] patient, some of my family, colleagues, etc and more pushing many avenues expressing things needed attention.

🌀🌀Fast forward Wayne’s World style 🌀🌀 led to extensive surgery of my FULL spinal fusion T5-L2. This is the area not far down from your neck, and down to the 2nd vertebra in lumbar. If anyone wants some screws, rods, or many other items, one day ask me and I’ll have, but the amount, I’ll ask the team for certain.

There’s truly some people I couldn’t have done any ounce of this without or continue to move towards next steps of being at my sister’s. My sister (Dana Rothbaum Roth), brother (Howard Rothbaum), and brother-in-law (Stuart Roth) have been working their butt off alongside some of the amazing people I introduced the other day. Without any of them doing their piece, all of this would be more of a fiasco than it is. Along with the above, one of my best friends (Brady Bolyard) of almost 30 years, I truly wish some of the heavy/song playing knocks on the door one day will be you, because we both know how much we both need that now more than ever. But to keep his ego in check, any knock brings me joy as long as it’s a helpful one. Even 30+ years into this IBD thing, you sadly still learn who’s really there at your most vulnerable & scared times, and who is nowhere to be found. I’m currently in a rehab facility & haven’t been in my bed in months.

I’ve been asked about pain — I haven’t achieved back pain relief since about April 2nd/or really March 26, except for the day of surgery (May 30) when they put me out. Otherwise, my pain has sat at around 9000/10 & the bottommost lobes on both lungs are still partially collapsed, likely due to the infection, which makes breathing overwhelming sometimes.

Dr. Artur Shifler - one of the kindest and attentive people to be involved in my case. I can’t continue to thank him enough, yet I will out of kinship separate from doctorhood.

It is extremely hard helping to push through my years of Crohn’s induced ARFID while in a facility. 💔 I’m doing the best I can in that arena of recovery.

Few closing detail reminders:
—— ADVOCATE as hard as you can even when you can barely breathe; stay involved/informed on your body’s state even if it feels intimidating.
—— SPEAK UP to anyone; you never know who’s listening.
—— Introduce yourself to Dr David Schwartzberg. He often dislikes me saying that this man and his mentor Dr. Feza Remzi have LITERALLY saved my life more than 4 times already 2025. DS - you know.
—— FRIENDS/ACQUAINTANCES don’t really care except maybe a handful. Don’t spend much time on them if there’s nothing filling your heart from them too.

The X-ray with “1” is the initial X-ray I saw when rolled into emergency surgery. It was around the room like wallpaper. They asked if they should remove and I said no, I need this to soak in.
The X-ray with “2” is fairly recent. You can see that T10/11 are gone and titanium pieces are in their original place.
**Serendipity: once I’m allowed, something that points to this word will be sketched onto me forever.

Overdue UPDATE:
3 months s/p / /stoma revision + thoracic osteomyelitis. Buckle up because it’s full & likely missing many, but I’ll pick them up soon.

Starting backwards: I’m fighting thoracic osteomyelitis with every being in me. A few more weeks of abx, then the next step, then I can start to really plan to get back home.
Reiterating: We have zero connection this horrid septic/bone infection is related to my Jan 29 extensive abdominopelvic surgery. The trail’s thought to have started as asymptomatic kidney infx going septic then taking seat in my known weakest part of my spine of T 9-12. No idea however I got onto that plane in Cincy & painfully flew with my sister to NYC that April night, but it LITERALLY saved my life.

Everyone knows although I live a fairly public disease life, I keep a whole lot of my life very private. There’s been a few in my pocket I’ve kept slightly under lock & key that have saved my life now 2 times in 3+ months. Before November I thought of them as friends and colleagues, some leaning more one way than the other, but all deserve public accolades. Although I still have a very long recovery extended due to this severe infection, it’s only fair I share some of my care team for others to be able to receive similar treatment if they’d ever need it. In the words of 1 of them, this is one of THE BEST DECISIONS we’ve made for your disease care in such a long time. Notice he said “we,” and I’ll say over & over, this man means we. When others would doubt me, he’d always say “hang in there,” we’ll get you better.

Cincy Team - Dr. Terry O'Toole - Gastro Health - Ohio almost 14 years running and banging our head, but the biggest is even during my notorious Dr vacays (I get burnt out from constantly seeing clinicians and giving too much attention to my own health), he knew I was willing to try almost anything until I wasn’t (in more recent years when I was getting too worn out). He’s always thought outside of the box along with me when I bring him the latest research he may or may not have seen from researchers I personally know. Along with my years of continuing to keep my medical records, although some years are painfully difficult to reread, I’ve kept them for myself but also I was dx prior to electronic records.
Becka RN - OC. She knew from day 1 of meeting me in January that we clicked and when I was scared, she jumped into action. Can’t wait for the weekend when I finally return.

Being a sometimes over-educated patient can be both scary knowing “too much,” but also intimidating to those learning. I don’t ever want to intimidate someone with any of my knowledge as long as we have the same end goal. I want to learn and be involved in my care, and depending on the day I tend to offer to have anyone interested on the floor to watch me/help me change my ostomy and learn port care. If there’s a resident during the Dr parade that may seem like they have an interest, I make sure it’s known to not hesitate to ask me anything. I oft remind I had to be taught by someone and it’s better to ask than make life harder or let pt lay in own excrement.

The NYC team - Dr David Schwartzberg, Dr. Feza Remzi, their immediate multi-d colleagues at Northwell Health IBD Center Lenox Hill, their Fellows, and residents, and especially the RNs/PCAs. The similar team quickly assembled at Northwell Health Northshore after Schwartzberg met my sister and I in ER and caught everyone up - I could only moan in pain with every breath - no talking, severely thirsty, high fever…
From Remzi running into my sister by chance, infectious disease getting a heads up I was coming, pain management, every resident calling Schwartzberg each shift because if you know DS, he was making certain no meds dropped off or were written differently to not change my regimen by any means. Every resident/Fellow knew to call Schwartzberg immediately - I’ve said I can’t imagine receiving the messages from me he’s received these months, but I follow it by I can’t wait for our convos to go back to our normal random not mainly me and my health. Remzi ordered as many tests as he could for such possible needs since some tests can wait days/weeks for the next open slot. These 2 brilliant minds coming together over me literally in OR (in January) and for this infection, brings me to literal tears. Although like Remzi said my last day inpatient, he can’t wait to go back to Feza/Alison the way we’ve known each other for years.

O’Toole - Georgie’s proud of you. We know it.
NYC - DS, you never gave up on me while knowing I was obstructing every day for ≈6y and fighting against my medical PTSD. Now Remzi and you are giving me a renewed life except with this infection detour.
KI - Everything I’ve told you along with DS screaming ALBUMIN in pre-op photos to you. If you don't know IBD/GI Dietitian, Kelly Issokson, she's the bestestest.

UPDATE: Post-op / surgery week 10

Have been progressing well including facing my ARFID head on (it’s incredibly hard), multiple days a wk of PT (including protocol my mentor & I wrote in 2012), Drs apps, etc.

03/27 (Friday): Out of left field I was down with 103.4 & bone shivers. Protocol’s are to discontinue TPN (just in case the line is the source) & start fast lactated ringers. Broke fever almost full 48h.
I said if I went a full 48 hours I’d chalk it up to something else. But, the spike returned and I requested cultures & my sis to Cincy STAT.

04/01 (Tuesday): CMP & WBC WNL —> weird
Cultures growing & fever persisting, but my back randomly became debilitating in a way it never had in all my years of known thoracic 9-12 issues.

04/03 (Thursday): by hair on my chin, we made the very torturous LGA flight.

04/04 (Friday): Admitted in AM & empiric abx awaiting culture specifics. But also, my CRS met me in ER to support me especially since I could barely communicate through the pain. My Gastro Health - Ohio GI closely in communication.

✨✨✨Get a CRS that’ll hold your hand for an hr in ED when moaning in misery. We deserve compassion.✨✨✨

Currently: inpatient day 10 (?)
Dx: osteomyelitis* - I remembered a little of it from my college & clinic days. Infectious disease (ID) said it’s one of most painful infections.
04/07 (Monday): Blood cultures neg, start count — 7d 🙌🏼
Treatment: 6w abx (count starts at last negative culture), 3-4w excruciating back pain.
Expect to discharge to CT mid-next wk

Due to pain, I can only communicate when meds allow.
Friday: 22y ileostomate anniv + my gpa’s would be bday. My bro washed my hair 1st x in 10d 😭 Likely his 1st x in 45y.

My Cincy Dr + sis & BIL are literal lifesavers for getting me this far so fast because they heard my pleas 😭🙏🏼 DSFRDH++
Still sometimes low grade fever, but nothing worrisome, they’re miserable because I get cold and bone shivers. Difficulty walking, snails pass me, shivers or not.

*caveat: This is NOT related to my major January surgery. Likely asymptomatic renal infection seated weakest area of T spine - known t spine’s been bad since I was 13
**Thank goodness the ostomy I changed on my bathroom floor with 103.4° lasted 12 days!!
***I don’t leave home without my fav fan and wipes.

🍔 Reminder: Needing medical supplies is tough. Paying for them (with/without insurance) is rough. Add on when insurance withholds medically necessary sanitary hazards, it escalates.

NEEDING ASSISTANCE with DME ordering while on HHC 🧑‍⚕️🌹

Anyone with experience being on home health care (HHC) and they say they don’t order DME (durable medical equipment) anymore?

Ex: I’m 7 wks post op. Still on TPN (hopefully not much longer!!) - receive (1) RN visit and (1) delivery per week.

In the past when on HHC they have to order DME, so I hold out as long as possible. I found out today this HHC/RN company doesn’t order DME anymore, but suppliers won’t even attempt to accept an order because I’m on HHC. Now what?

*New stoma can’t work with previous wafers 😏 So I’ve been getting by with samples.

I’m not sure if people still see this page, but just in case, I wanted to share an update.

First and foremost, thanks to everyone who has supported me through my long journey with IBD (Crohn’s) and my oft tireless efforts in IBD/ostomy advocacy, as well as other important healthcare advocacy.

This path has taught me so much—not just for myself but with the hopes of sharing knowledge with others, whether patients or physicians. Along the way, I’ve been fortunate to connect with incredible leaders in the field—both patients and clinicians—some of whom are truly friends.

For the past 14+ years, I’ve had an open-minded and compassionate GI who has taken on my complex and refractory case. Despite countless obstacles, he’s never given up on me or the broad range of ideas and random research I bring his way. His unwavering support has meant the world to me, and it’s a reminder of how crucial bedside manner is, no matter how jaded the system can be.

Through advocacy, especially on platforms like Twitter, I’ve connected with countless specialists—both internationally known and unsung heroes in IBD care. While I’m very public about some aspects of my life and health, there’s much I keep private, and I’ve become adept at compartmentalizing. Yet, some people see through what I try to hide and have continued to encourage me to pursue the care I need.

A significant hurdle has been overcoming my mental health struggles, which have made it difficult to push past dismissive experiences and seek in-depth care. But when I hit my breaking point, I found myself begging for relief.

Now, with the incredible support of my GI and a lot of coordination, I’m preparing for surgery in about 2 weeks. This will include refashioning my nearly 22 year old stoma, necessary resections, adhesion cleanup, and, most importantly, I have renewed hope—something I haven’t felt in years.

I’m scared, of course, but I’m also incredibly grateful for the surgical and dietary team that’s already been working to help me. The road ahead won’t be easy, especially as I reintroduce solid foods I’ve been craving (looking at you, tomatoes). But it all starts with a celebratory sliver with some of my fav big whigs—and another hero joining us long-distance.

I’ll try to document what I can, but don’t be surprised if it’s a post-op gibberish fest. Here’s to hope, progress, working epidurals, lots of the good stuff, friendships, siblings, and seeing you on the flip.

We are responding to Hurricane Ian, which made landfall in Cayo Costa, FL as a Category 4 hurricane yesterday and has left many areas submerged in floodwaters and more than 2.5 million homes and businesses without power. Our Disaster Response Team is deploying to provide relief supplies and services to those affected and we will be shipping additional truckloads of aid. Ian was downgraded to a tropical storm this morning and will continue to bring extreme rainfall, catastrophic storm surge, and intense winds as it moves across the state and north towards Georgia and South Carolina.
