At MISI Community Solutions, our Enhanced Care Management (ECM) program is designed to support Medi-Cal members through life’s most complex health and social challenges.

✨ With ECM, you may receive:
• A dedicated personal health coordinator
• Help scheduling medical appointments and managing medications
• Support with housing-related needs
• Assistance with transportation and interpreter services
• Guidance during complex or high-risk situations

ECM may be available for Medi-Cal members experiencing chronic health conditions, serious mental health challenges, high-risk pregnancies, frequent ER visits, or housing instability.

MISI Community Solutions, we’re here to support you and your family during important moments of life.

Through Housing Navigation, we can assist eligible Medi-Cal members with finding housing and accessing move-in support such as deposit assistance and basic furnishings, when authorized.

Our Enhanced Care Management (ECM) services help coordinate your care — including appointments, reminders, transportation, and interpreter services — so you feel supported every step of the way.

We also offer Doula services for eligible expecting and postpartum mothers, providing guidance and support during pregnancy and after birth.

With help from our Community Health Workers (CHWs), you can get support navigating the healthcare system and connecting to community resources that matter most.

If you live in Fresno and need help with housing or health-related support — we’re here for you.

At MISI Community Solutions – Fresno Office, we provide CalAIM Community Supports for eligible Medi-Cal members who meet program criteria.

We can assist with:

▫️ Housing Support — help finding affordable housing, move-in assistance, basic furniture, cleaning support, and tenancy support (when authorized)
▫️ Enhanced Care Management (ECM) — coordination of care, help with appointments, transportation, meals, and interpreter services
▫️ Community Health Workers — support with benefits, healthcare navigation, and everyday needs
▫️ Doula Services — support during pregnancy and the postpartum period
▫️ Recuperative Care — short-term housing and recovery support after hospitalization (when authorized)

- We’re Here to Help.

At MISI Community Solutions, we provide Housing Assistance as a CalAIM Community Support for Medi-Cal members who meet eligibility criteria.

Our Housing Assistance services can help eligible members with finding and maintaining stable housing, including:

• Support with housing search, applications, and lease signing
Assistance with move-in needs such as security deposits, basic furniture, and utility connections when approved through the member’s Medi-Cal health plan
• Ongoing housing stability support and coordination with landlords to help prevent housing loss

These services are designed to support Medi-Cal members who are experiencing homelessness, are at risk of losing housing, or have health conditions that make stable housing essential.

CalAIM Community Support Services at MISI Community Solutions

MISI Community Solutions provides CalAIM Community Support services for Medi-Cal members who meet eligibility criteria, including:

▫️ Housing Assistance
Support with move-in needs, including help with a security deposit and basic furniture when approved through the member’s Medi-Cal health plan.

▫️ Enhanced Care Management (ECM)
Support with medical needs — from scheduling doctor’s appointments to receiving medically tailored meals.

▫️ Community Health Worker (CHW) Services
Help with a wide range of social and daily living needs, including navigating healthcare and community resources.

▫️ Doula Services
Support during pregnancy and the postpartum period for members who qualify.

▫️ Recuperative Care
Short-term recovery support after hospitalization, including temporary housing and meals for members who do not require hospital-level care.
