PACE of Southwest Michigan The Program of All-Inclusive Care for the Elderly (PACE) helps older adults remain at home by providing integrated medical and social services.

PACE (Program of All-inclusive Care for the Elderly) is a unique program of long term care that allows elderly adults to remain in their home. We work with you and your loved ones to provide the support at home, medical treatment, and social interaction needed to remain strong and independent. The key to your success is our care team. This is a team of physicians, nurses, pharmacists, social workers, therapists, and transportation staff who are familiar with you and your needs. The care team meets daily to discuss all aspects of your care and provide services when appropriate. Our physician is a geriatrician specializing in the care of adults as they age. We encourage caregivers to remain involved by providing frequent communication and supportive services including respite care. PACE of Southwest Michigan serves Berrien, and parts of Cass and Van Buren Counties. Note: A PACE participant may be fully liable for the cost of medical services from an out-of-network provider or without previous authorization (except for emergency services).

March is Social Work Month, and we are proud to celebrate our incredible Social Work team!

Our social workers do so much more than psychosocial assessments. Every day they serve as advocates, counselors, educators, and compassionate supporters for our participants and their families.

Their work includes:
• Cognitive and mental health screenings
• Advanced Care Planning
• Active participation as members of the interdisciplinary care team
• Providing emotional support to patients
• Conducting home visits
• Supporting families and caregivers through education and support groups
• Coordinating resources such as housing assistance, documentation renewal, and Medicaid redeterminations/FAP applications
• Counseling and hospice referrals
• Placement coordination for Skilled Nursing Facilities (SNF) and Adult Foster Care (AFC)
• Advocacy for patient needs
• Crisis intervention during difficult moments

Our social workers meet people where they are—helping navigate complex systems, supporting families through challenging transitions, and ensuring our participants receive the care, dignity, and resources they deserve.

March is Reading Mont! Join us at one of our Memory Cafes with your favorite book in hand. “Book” is defined broadly as cookbooks, books of photos, sports card collections, novels, history books, etc. We'll also create a bookmark!

PACE was created to provide you, your family, caregivers, and healthcare providers the flexibility to meet your healthcare needs and help you continue living in the community.

At the PACE center, participants enjoy: Primary care and therapy, Nutritious meals, Recreation and socialization, Personal care

At home, PACE provides: Skilled care and personal care support, Safety tools like ramps and grab bars

In the community, participants gain access to: Specialists and additional healthcare providers

PACE combines the personal touch of a dedicated provider with the coordination and efficiency of a health plan. Our interdisciplinary team delivers much of the care directly, so we can closely monitor participants’ health and respond quickly to any changes.

We had such a wonderful and eventful Valentine’s Day with our participants!

The day began with participants having the opportunity to create their own Valentine’s cards. Everyone enjoyed getting creative and making heartfelt messages for friends and loved ones.

Throughout the day, we’ve had even more fun activities planned. Participants decorated delicious cookies and will have additional chances to decorate sweet treats later on. This afternoon, we’re wrapping up the celebration with a fun and engaging “Who Is Most Like Cupid?” scoring game, where participants can showcase their playful and loving personalities.

It’s been a full day of creativity, laughter, and celebration, and we’re so happy to share these special moments together!

NuStep Challenge Highlights – 2025

In 2025, our participants took on the NuStep Challenge, competing in creative, themed races throughout the year—all while staying active and having fun!

Total steps completed in 2025: 6,496,838 steps!

Here’s a look at the races and challenges that made this year so special:

We kicked things off by finishing our winter-themed bobsled race down Candy Cane Lane.
For Valentine’s Day, participants became Cupid—using Nerf dart guns to shoot hearts and earn bingo bucks.
Next came a board-game-style Easter challenge, helping the Bunny reach his basket (with a few tricky holes that sent him back to the start!).
Our biggest event was the baseball game—each participant had their own player. Every NuStep ride earned a spin of the wheel to move their player toward home plate. The player with the most points won!
In the fall, we focused on fall prevention, where participants drew a leaf and shared ways to prevent falls at home.
Hunting season followed, with Nerf dart target practice—shooting deer, turkey, pumpkins, and targets to earn bingo bucks.
To wrap up the year, we challenged ourselves to get Santa from the North Pole to St. Joseph, Michigan—and we did it! Thanks to Santa’s magic, the travel time was cut in half.

We’re so proud of our participants for their dedication, creativity, and teamwork all year long.
