Focusing on self-directed services in Minnesota, the Self-direction Connection Conference invites individuals with lived experience to share their knowledge with each other. As a first-of-its-kind event in Minnesota, attendees will gather to discuss trends and their experiences with Consumer Directed Community Supports (CDCS) and the Consumer Support Grant (CSG).

Meet others using CDCS and CSG and connect with them.

The conference will be held at the DoubleTree by Hilton Airport Hotel in Bloomington on October 22nd.

We are for a Designated Coordinator at St. Paul, MN.

Life Fountain is currently seeking a motivated and compassionate entry-level professional to join our home healthcare team as a Supervisor. Our agency is currently licensed to provide Personal Care Assistance (PCA), Community First Services and Supports (CFSS), and 245D services.

A professional in this role would be responsible for carrying out the functions of a supervisor for direct care staff providing PCA/CFSS and 245D services. As a Supervisor, you will play an important role in ensuring service recipients get the care they need by coordinating staffing and service activities, hiring and training staff, and addressing any recipient or staff concerns. The Supervisor would be serving as a QP/SP and a DC to ensure coverage for PCA, CFSS, and 245D services.

Must meet minimum requirements in one of the following ways:

1) Bachelor's degree in a field related to human services, education, or health, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children

2) Associate degree in a field related to human services, education, or health, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children

3) Diploma in a field related to human services, education, or health from an accredited postsecondary institution AND three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children

4) a minimum of 50 hours of education and training related to human services and disabilities; and

5) four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications listed in 1) - 3)

We are looking for a bilingual (Spanish/English) candidate, preferably.

Monday-Friday, no nights or weekends
9am to 5:30pm
$20-24/hour DOE

The state of Minnesota is conducting a survey to learn more about direct support professionals to better understand and improve working conditions for this labor market.
The survey takes just a few minutes to complete. The survey is available until 5 p.m. Friday, Aug. 29.

Direct support is critical work provided by people with many different titles, such as home health aides, personal care assistants, nursing home aides, and others. They help people with disabilities and older adults with activities of daily living like eating, dressing, and using the bathroom.

If you know someone who does this important work, please encourage them to take the survey.
