I recently had a client request the preservative free option for vitamin k. When the nurse returned with it, she said "actually it says free of added preservatives. Not preservative free." (Big thanks to her for saying that)
That sparked a lot of curiosity so, when I returned to the same location a couple of weeks later, we asked to see the label and take a pic of it.
Then I went to Chatgpt to find out. Does the preservative free option even exist? And this is what I got.
Standard Vitamin K1 (phytonadione) injection for newborns in the U.S. typically contains a small amount of preservatives (like benzyl alcohol).
Preservative-free formulations do exist. These are usually labeled as “preservative-free” (PF) or “without preservatives”, and are packaged in single-use vials or ampules (so they don’t need preservatives for multi-dose stability).
Sometimes you’ll also see “no added preservatives”, which can mean the drug itself is formulated without preservatives, but you still need to check the ingredient list — some products use stabilizers like polyoxyethylated castor oil (not technically a preservative but sometimes confused as one).
So, to be precise:
✅ Yes, there are truly preservative-free vitamin K shots (especially those labeled for neonatal use).
⚠️ But not every “no added preservative” product is automatically preservative-free — you have to read the manufacturer’s label or ask the pharmacist to confirm the exact formulation.
