Too many times I’m seeing patients who have a benign nodule and want RFA but may not be ideal candidates. Location, location, location matters. The surrounding tissues need to be spared from the intense heat of the probe tip. You will hear terms like the “danger triangle.”
If the nodule is beneath the clavical only the portion readily seen above the clavical is “safely ablatable.” The mass shrinks and brings the under portion in view for a second procedure 3-6m later.
Ideal size is 2.5-3.5 cm. Larger nodules may require a second or even a third procedure.
Evaluating the caliber of the trachea is vitally important. Some masses are so firm and large they not only cause shift of the trachea, they may compress it and narrow the airway passage. If the trachea is narrow, minor swelling post procedure may further impair air movement to your lungs.

Sean Underwent RFA for a large benign thyroid Nodule encompassing most of his right lobe extending under his clavicle causing symptoms of compression, choking and pressure. Hot or autonomous nodules, produce an abundance of thyroid hormone that may cause hyperthyroidism (overactive thyroid). New standard for treatment is RFA over surgery, I 131 or chronic ATM (antithyroid medication). Patients are typically cured within a week to month after procedure.
