The Centers for Medicare & Medicaid
Services (CMS) has launched a new Dementia care management program called GUIDE (Guiding an Improved Dementia Experience). This program is designed to improve the quality of care and support for people living with dementia and their caregivers.

Visiting Practitioners & Palliative Care LLC is a proud participant in the GUIDE Model.
