"Can I still make progress if it's been more than a year after having a stroke?" 💭

I get this question a lot with my patients in my mobile Physical Therapy practice. And the answer is yes you can definitely make progress years or even decades after your stroke. The old adage or thought was that after about a year all progress would be stopped at that point. The reason for that a lot of times is because most patients will not receive therapy past roughly 6 months to a year after their stroke.

You definitely can regain partial movement in affected extremities and progress with many different functional activities whether it be just sitting up at the edge of the bed, transfers, walking or anything else that they might do during their day.

This can be extremely exciting for you and your family as you continue to navigate your life post stroke.

I will be describing a little bit more in detail how this occurs in future posts but the term that we used to describe that is brain plasticity or neuroplasticity. Simply defined it is the ability of neural networks in the brain to change through growth and reorganization.

I look forward to talking about stroke recovery along with other neurological issues that affect adults. Please feel free to reach out and send me a message or comment on this post and I would love to answer any and all questions as they come in.

👨‍🏫Pain is complex. It can be confusing because there isn’t always a specific mechanism of injury. It can be debilitating because it limits you from performing the activities you want to do. And it can be challenging to find the proper path of recovery.

💊 There isn’t a quick fix unfortunately. There’s no magic pill that’s going to instantly resolve all of your issues. Fortunately, most pains usually do get better and these are some of the ways to help facilitate the process.

✅Focus on what you CAN do. CANs facilitate hope, positivity, and progress. CAN’Ts help set goals initially, but too much emphasis on them hinders the recovery process. I need to see what you CAN do so we can start working towards the things you CAN’T do.

🏃‍♂️Set goals that are Specific, Measurable, Attainable, Relevant, and Time Based (SMART). You want to get out of pain and I want the same for you. However, we need more depth than that. You don’t visit a doctor or physical therapist every time you’re in pain (stubbed toe, headache, sore shoulder). You make an appointment when that pain begins to limit you from doing certain activities comfortably. Therefore, most goals should be based on function. And when your function gradually improves, I promise you’ll start to notice your pain less and less.
🧠Mindset is also key and is usually not the fault of the individual. Understanding that your body has the inherent ability to heal itself with the proper guidance is much more powerful than thinking that you need to be fixed by someone else. It’ll lead to more successful outcomes immediately and down the road.

The reality is, if you truly wish to succeed, if you truly wish to do monumental things with your life, you are going to have to suffer.

Many days are to come, where you will be exerted emotionally, psychologically and physically.

There will be moments where you are unmotivated, tired and hurt.
-
There will be moments where you just think to yourself you can’t do it anymore.
-
During your journey, you will also be compelled to face things that you do not want to do, because they appear to be too difficult and too different.
-
You will not be granted the luxury of doing everything your way and only things that you want to do.
-
Your path will not be straight, and you will have to navigate diligently, for that is the only way you can make it to the other side.

You must continuously do things which pose as a challenge to you, you must do things that you do not want to do, and you must persist, you must stick with it.

In the end, it will all be worth it, all your time, all your effort, all your sacrifices and all your hardwork.

ACL Rehab Protocols: Which is Best?

BACKGROUND:

Many orthopedic protocols following ACL reconstruction exist, however they all differ slightly in quality and content.
ℹ️
Makhni et al. (2016) assessed both the quality and variability across ACL rehab protocols published by academic orthopedic programs.
ℹ️
Online ACL physical therapy protocols from the United States were included.
ℹ️
Each protocol was assessed for inclusion/exclusion of various rehabilitation components and the timeframe for when those activities were initiated in each protocol.

RESULTS:
42 protocols were included for review.
📈
Of the 42 protocols reviewed, recommended components included:

Pre-habilitation programs: 13 protocols (31%).
📈
Post-op Continuous Passive Motion: 5 protocols (12%).
📈
Routine partial or non–weight bearing immediately postoperatively. 11 protocols (26%).
📈
Utilization of a secondary/functional brace: 10 protocols (24%). .
📈
Return to sport only after achieving specific strength and activity criteria: 8 protocols (19%) .
📈
Considerable variation existed in:
Time for desired full-weight-bearing initiation (9 weeks).
Types of strength and proprioception exercises specifically recommended. .

CONCLUSIONS:

Many of the reviewed ACL rehabilitation protocols recommended treatment modalities the current research doesn’t support.

Importantly, a high variability in content and time scales of rehab components existed, which can be confusing to both patients and therapists.

💥In cases of low back pain (LBP) where pain also radiates into the leg (radicular pain), one of the goals in rehabilitation is to determine if the patient's symptoms can be reduced by placing the spine in a particular position (flexion or extension-swipe left to see videos of these positions). If a position is found that reduces pain, this is referred to as the patient's directional preference and is used as a guideline when prescribing exercise/movement.

🔎When radiating pain is present, it is important to understand that if pain leaves the leg and moves closer to the spine (centralization phenomenon), this is a sign of neural desensitization and recovery. On the other hand, if pain spreads farther away from the spine (peripheralization), this is a sign that the condition is worsening.
-
✅If you are experiencing LBP that radiates into the leg, it can be very helpful to see if you can find your own directional preference and, at the same time, identify any positions that worsen symptoms. In the picture shown here, this patient may have reported increased symptoms when in spinal flexion (sitting, bending to the ground, etc) and reduced pain with extension (laying on their stomach, standing, etc)

💥In this case (which is actually quite common), extension would be this individual's directional preference and they would be advised to avoid positions of prolongedor repetitive flexion and use extension positions or repeated movements (i.e. standing and arching the low back 10-15 times) to create centralization and speed recovery.
