According to LeadingAge, half of U.S. counties have five or fewer home health agencies per 1,000 square miles — and in many rural areas, only one agency, or none at all, serves more than ten patients. This stark reality leaves thousands of seniors, people with disabilities, and chronically ill individuals without the essential care they need.

🚨 It’s Official: The One Big Beautiful Bill Act Is Now Law — and It Puts Home & Community-Based Care at Risk 🚨

On July 4, 2025, the One Big Beautiful Bill Act was signed into law, despite urgent warnings from healthcare leaders, including the National Alliance for Care at Home.

While aiming to cut federal spending, it threatens vulnerable populations — especially older adults and people with disabilities who rely on Home and Community-Based Services (HCBS).

💬 Dr. Steve Landers, CEO of the Alliance, warns:
“This law prioritizes budget cuts over the health of our most vulnerable… Cutting over $1 trillion in Medicaid expenditures will impact services.”

Key concerns:
📉 Cuts to Medicaid HCBS funding
🧾 Cost-sharing and administrative burdens
⚠️ Broader ripple effects on Medicaid services

HCBS keeps people safe at home and improves outcomes. This law threatens that.

As home care providers, we’re closely watching the new CMS WISeR (Wasteful and Inappropriate Service Reduction) Model, set to launch in January 2026. While its aim to reduce fraud, waste, and abuse is commendable — and essential to maintaining the integrity of care delivery — the implications for access, timeliness, and transparency in services like home health remain top of mind.
The model, which introduces prior authorization to select Medicare fee-for-service items, will begin in five states and exclude high-risk or time-sensitive services. Still, many in our field know how prior authorization, especially in Medicare Advantage, has contributed to care delays and abandonment, particularly for vulnerable home health patients.
It’s promising that CMS is exploring AI-driven solutions to streamline the process, but concerns around transparency and care barriers remain real. Collaboration between providers, policymakers, and technology partners will be critical as this model evolves.

Access to home health care in rural areas goes beyond counting agencies—it’s about capacity and workforce. Many rural providers can’t accept new patients due to staffing shortages and long travel times between homes. Operating on thin margins, often as nonprofits or hospital-based units, they face financial strain. The workforce crisis is deepening, with burnout and low wages ($14.51/hr median) driving aides out. As demand grows with an aging population, the rural care gap will widen without urgent policy reforms to support and stabilize this vital workforce.
