Regional Director of Specialty Programs, Mercedes Martinez, MSN, RN, met with the Chestnut Woods interdisciplinary team to introduce and educate staff on the AHA Heart Failure Specialty Program accreditation process.

Education included Heart Failure assessments, daily weights, daily vital signs, and patient education at discharge. The session emphasized interdisciplinary roles and expectations to support consistent workflows and improved patient outcomes.

Chestnut Woods has officially entered the AHA Heart Failure accreditation process, marking an important step in advancing specialty program development within the facility!
