The Supports Broker will primarily assist with
employer-related functions, planning, budgeting, accessing
community resources and maintaining supports and support networks.
Who Qualifies for Supports Brokering?
People who self-direct their services and supports, and are limited in the scope to assisting people with activities of self-direction. The individual must be using self-directed serv
ices under the Consolidated or P/FDS Waivers. Support Broker services are optional, and are directed based on the individual's specific needs, the areas where brokers can assist a person are:

Services are provided throughout Lackawanna, Luzerne, Susquehanna, Wayne, and Wyoming Counties.
Enhancing Natural Supports and Community Connections Supports Brokers can work as a networker to search out and obtain supports within the community. Such as social clubs, jobs, or volunteer opportunities. The Broker can help guide and coach the person in building and sustaining a network of paid and unpaid people. Communication and Advocacy
Supports Brokers can work together with the person, their families, the Supports Coordination agency, as well as government or community supports to prevent breakdowns in communications. Supports Brokers can mediate and collaborate with all involved parties and work as a problem solvers and networkers to find solutions for the person and their families. Supports Brokers can attend ISP meetings and take an active role in determining and defining goals for the individual by clarifying their needs and wants.
