WHY ARE PRESCRIPTION DRUGS ARE SO EXPENSIVE? PHARMACY BENEFITS MANAGER (PBM)

DEAR PRESIDENT TRUMP, ELON THE CON MAN< AND RFK Jr., here’s an easy fix to save tens of billions of dollars. Eliminate PBMs or Pharmacy Benefit Managers. A bureaucratic mess to make middlemen wealthy.

Unfortunately, I grew up in Canada, and their healthcare is among the worst in the world. However, their medicine prices are vastly cheaper. Why? They tell pharmaceutical companies what they will pay for the medicine.
The Role and Controversy of Pharmacy Benefit Managers: A Call for Reform

Abstract: Pharmacy Benefit Managers (PBMs) serve as powerful intermediaries in the U.S. pharmaceutical supply chain. Initially designed to control drug costs and improve access, PBMs have evolved into opaque entities accused of driving up drug prices, reducing transparency, and undermining healthcare affordability. This paper critically explores the function of PBMs, their financial practices, and the growing demand to eliminate or reform these middlemen to save taxpayers, Medicare, and consumers tens of billions of dollars annually.

1. Introduction
Prescription drug costs in the United States continue to outpace inflation and global benchmarks, with Americans paying the highest prices in the world. At the center of this issue are Pharmacy Benefit Managers (PBMs)—corporate intermediaries that negotiate drug prices between pharmaceutical manufacturers, insurance providers, pharmacies, and employers. While their original intent was to streamline operations and lower costs, PBMs have become some of the most profitable and least transparent entities in healthcare.

2. What Do PBMs Do?
PBMs operate as third-party administrators of prescription drug programs for health plans, including employer-sponsored insurance, Medicaid, and Medicare Part D. Their key functions include:
• Negotiating rebates and discounts with drug manufacturers
• Creating formularies (lists of covered medications)
• Processing and paying prescription drug claims
• Operating mail-order pharmacies
• Determining reimbursement rates for pharmacies
Initially, PBMs were meant to act in the best interests of health plans and consumers by leveraging buying power to lower drug prices. Today, however, their incentives are often misaligned.

3. Why Are PBMs So Controversial?
3.1 Lack of Transparency PBMs operate under non-disclosure agreements and do not fully disclose how much of the manufacturer rebates or discounts are passed on to consumers or health plans. This opaque model makes it impossible for payers to verify if PBMs are acting in good faith.

3.2 Spread Pricing One of the most criticized practices is "spread pricing," where PBMs charge insurance plans significantly more than they reimburse pharmacies for the same prescription. The PBM pockets the difference.

3.3 Rebate Schemes Drug manufacturers pay PBMs large rebates to secure favorable placement on drug formularies. This creates a perverse incentive to prioritize higher-cost drugs over cheaper, equally effective alternatives.

3.4 Vertical Integration Major PBMs like CVS Caremark, Express Scripts, and OptumRx are now owned by large insurance companies or healthcare conglomerates. This consolidation enables them to profit at multiple levels of the supply chain, from insurer to pharmacy.

3.5 Impact on Independent Pharmacies PBMs reimburse independent pharmacies at rates so low that many cannot stay afloat. Meanwhile, vertically integrated PBMs funnel more business to their own mail-order or chain pharmacies, further skewing the market.

4. The Cost to Medicare and Taxpayers
Numerous studies and government reports indicate that eliminating or regulating PBMs could save tens of billions annually. For example:
• A 2021 Congressional Budget Office report estimated that direct Medicare drug negotiations, eliminating PBM middlemen, could save over $45 billion over ten years.
• A 2022 Senate Finance Committee report found that PBM practices were inflating drug costs and stifling competition, particularly in the Medicare Part D program.
• According to research by the West Health Policy Center, eliminating PBMs entirely and adopting a more transparent, direct-purchasing model could save up to $100 billion over a decade, with estimated annual savings exceeding $10 billion.

• State-level audits, such as those conducted in Ohio and Kentucky, revealed hundreds of millions in overcharges within Medicaid managed care programs attributed to PBM spread pricing.

These figures suggest that the U.S. could save substantial amounts by reforming or removing the PBM model.

5. Why Not Cut Out the Middlemen?

5.1 Direct Negotiation Models Countries like the UK and Canada do not use PBMs. Instead, their governments negotiate drug prices directly with manufacturers. This model has resulted in dramatically lower drug prices. I grew up in Canada, and medications are really cheap.

5.2 Transparency and Public Oversight Reforms could require PBMs to disclose:
• Rebate amounts and their distribution
• Spread pricing practices
• Formulary decisions and associated incentives

5.3 Public Options and Government-Owned PBMs Some policymakers suggest creating a public PBM or allowing Medicare to negotiate drug prices directly, effectively bypassing the existing PBM system. California has already launched CalRx, a state-sponsored drug purchasing program aimed at reducing costs.

6. Legislative Efforts and Resistance
Several bipartisan bills have been introduced to regulate PBMs, including:
• The Pharmacy Benefit Manager Transparency Act
• The Drug Price Transparency in Medicaid Act
However, lobbying from the PBM industry, backed by powerful corporations, has stalled or watered down many of these reforms. The Pharmaceutical Care Management Association (PCMA), the lobbying arm of PBMs, spends millions annually to influence policy and public opinion.

7. Conclusion: A System Ripe for Overhaul
Pharmacy Benefit Managers have morphed from cost-controlling intermediaries into profit-driven gatekeepers. Their opaque practices, misaligned incentives, and market consolidation contribute significantly to the unaffordable state of prescription drugs in the U.S.

Eliminating or fundamentally restructuring the role of PBMs could save Medicare and taxpayers billions each year. Real reform will require political will, legislative action, and a shift toward transparency and accountability in pharmaceutical pricing.
We should tell the drug companies what they can charge. It costs about $800 million to $1.2 billion to develop a new drug.
It is estimated that savings from federal and state-level reforms with figures reaching up to $100 billion over a decade, or more.

MAKE AMERICA SMART AGAIN

MASA Condemns FDA’s Politically Motivated Ban on Red Dye 3
Make America Smart Again (MASA) strongly opposes the FDA’s recent decision to ban Red Dye 3, a move driven by political pressure rather than scientific evidence. The justification for this ban is deeply flawed, stemming from outdated rodent studies that do not apply to humans. By allowing fear-based activism to influence regulatory decisions, the FDA undermines its own credibility and betrays the public trust.

MASA demands a return to evidence-based policymaking, where regulatory actions are based on scientific rigor, not political agendas. The American people deserve policies grounded in fact, not reactionary decisions influenced by unqualified social media influencers.

The Truth About Red Dye 3
The FDA’s January 15, 2025, ban is based on the Delaney Clause of the Federal Food, Drug, and Cosmetic Act of 1958, which prohibits approval of any food additive that has been shown to cause cancer in any animal at any dose. However, this policy ignores a fundamental scientific reality: humans are not rodents.
Flawed Science: High-Dose Rodent Studies Are Misleading
Regulatory decisions should be based on real-world human exposure levels, not exaggerated animal studies. The primary study behind the Red Dye 3 ban involved rats consuming the equivalent of 4% of their body weight in the dye every single day for 19 weeks. To put this into human perspective:

• A 70-kilogram (154-pound) adult would have to consume 2.8 kilograms (6.2 pounds) of pure Red Dye 3 daily to match the test conditions.
• This is 14 million times the average daily intake for humans.
• Additionally, the biological mechanism that led to tumors in rats does not exist in humans, rendering the findings irrelevant.
This underscores the flaws of relying on extreme-dose animal studies to shape public health policy. Such reckless decision-making sets a dangerous precedent, where scientific nuance is ignored in favor of alarmist activism.

The Influence of Uneducated Activists
The FDA’s ban was not driven by science—it was the result of pressure from anti-science influencers like Vani Hari (The Food Babe) and Courtney Swan (Real Foodology). These individuals have no scientific training and routinely peddle misinformation for profit.

MASA’s team of qualified doctors and scientists has repeatedly debunked their false claims, yet instead of engaging in legitimate debate, these influencers block and silence anyone who challenges them. Their fear-mongering is not about health—it’s about control and financial gain.
The Reality: Red Dye 3 Is Safe and Approved Globally
Unlike the FDA, other leading regulatory agencies have taken a science-based approach:

• Health Canada recently reaffirmed that Red Dye 3 poses no risk to human health.
• The European Food Safety Authority continues to approve it under the name Erythrosine (E127).
• Dozens of other nations also permit its use in food and pharmaceuticals.
This raises a crucial question: If Red Dye 3 is truly dangerous, why do Canada, Europe, and other advanced nations still allow it? The answer is clear—it isn’t dangerous.
MASA Condemns Misinformation Spread by Media and Public Figures
MASA strongly condemns the following individuals and organizations for perpetuating falsehoods and damaging public trust in science:

1. Vani Hari (The Food Babe)
Hari has no formal education in science, nutrition, or medicine, yet she routinely falsifies information to promote her own brand. Her outright fabrications have been thoroughly debunked by MASA’s team of experts. Her approval of the FDA’s decision is meaningless, as she lacks the expertise to make any credible statement on the matter.

2. Bill Ackman
While MASA respects Ackman’s financial acumen, his statement supporting the Red Dye 3 ban exposes his ignorance on scientific and regulatory matters. This is not his area of expertise, and his comments reflect a fundamental misunderstanding of the science behind food safety.

3. FOX News & Media Figures
MASA condemns Jesse Watters, Laura Ingraham, and Sean Hannity for amplifying uneducated influencers like Vani Hari aka
The Food Babe. Their irresponsible promotion of fear-based narratives misleads tens of millions of Americans.
Additionally, Dr. Drew, a once-respected medical professional, has tarnished his credibility by endorsing fraudulent companies like VShred, which peddles junk supplements under false pretenses. Shame on them all.

MASA: Fighting Back Against the MAHA Movement and RFK Jr.
MASA was founded to combat the lies and misinformation of the Make America Healthy Again (MAHA) movement, which is led by unqualified, fear-mongering activists like:
• Vani Hari (The Food Babe)
• Casey & Calley Means (who have no expertise in public health, despite positioning themselves as authorities)
• RFK Jr., who is entirely unfit to lead the Department of

Health and Human Services
Over 17,000 doctors and scientists have signed a formal letter demanding that RFK Jr. be removed. His appointment is an insult to the medical and scientific communities and a slap in the face to the American public.

President Trump’s Grave Mistake
President Trump failed America by nominating RFK Jr. for HHS Secretary. This decision proves that Trump does not care about the health and well-being of Americans. Any Senator who confirmed RFK Jr. shares the blame for putting an unqualified conspiracy theorist in charge of critical health policies.

A Personal Stand for Truth
For full transparency, I voted for President Trump. However, I personally reached out to him via Truth Social, warning that RFK Jr. is a disastrous choice. He ignored my concerns, and now the American public is suffering the consequences.
Final Demand: Fire RFK Jr. and Disband MAHA Immediately
RFK Jr. must be fired. Now.

The MAHA movement must be dismantled before it causes irreversible harm to public health policy.
Liars cannot solve real problems—they only create more.
The American people deserve truth, transparency, and science, not deception, fear, and pseudoscience.

RFK JR. CONTINUES HIS CAMPAIGN OF LIES & MISINFORMATION
April 25, 2025

Food Dyes and “Eating Petroleum”: The Truth Behind the Outrageous Myth

In recent weeks, social media has been flooded with claims that popular food colorings like Red 40 and Yellow 5 are nothing more than “eating petroleum.” But a closer look at how these dyes are made and tested reveals a very different story.

RFK is continuing his parade of lies after lies. You’re not consuming petroleum, which shows your ignorance and lack of education. Just outrageous. Then the ignorant in this country then claim we are eating gas or oil. RFK Jr. is just a vile human being. Here’s a description of how things work.

This has only perpetuated this lie by saying things like this, RFK Jr., who said Americans are tired of eating petroleum. Another outrageous comment, which is another lie. To think we are consuming petroleum is again outrageous.
From Crude Oil to Chemical Building Blocks

Many synthetic dyes indeed begin their journey in the oil refinery. When crude oil is processed, it is separated into gasoline, kerosene, diesel, and a slate of petrochemical intermediates such as benzene and toluene. These molecules serve as the raw ingredients for countless products, from plastics to pharmaceuticals—and yes, food colorings.

However, the term “petroleum” in the refinery sense refers to the unrefined mixture of hydrocarbons in crude oil. What ends up in your food is not raw oil, but highly refined chemical feedstocks that bear no resemblance to the barrel from which they came.
The Chemistry of Color

Once refined, those intermediate chemicals enter the laboratory, where they undergo a precise series of reactions—known as diazotization and azo coupling—that stitch them into the vibrant dye molecules consumers recognize:
• Diazotization transforms aromatic amines into reactive intermediates.

• Azo coupling joins those intermediates to a partner compound, creating the distinctive “azo” bond that gives these dyes their intense color.

These steps do not merely tint the petrochemical feedstocks; they reconfigure them into wholly new, single-molecule substances—Allura Red AC for Red 40 and tartrazine for Yellow 5.
Purification: From Lab Bench to Dinner Plate
After synthesis, the crude reaction mixture is far from ready for food use. It must pass through multiple stages of washing, filtration, and crystallization to remove any unreacted starting materials or by-products. What remains is a powder or solution composed almost entirely of the pure dye molecule, nothing like the unprocessed hydrocarbons of crude oil.
Laboratory testing ensures that at least 85–90% of the final product is the target dye, with any impurities reduced to trace levels well below safety thresholds.

Rigorous Safety Reviews
Regulatory agencies around the world—including the U.S. Food and Drug Administration (FDA), the European Food Safety Authority (EFSA), and the World Health Organization’s JECFA—have evaluated these purified dye molecules extensively. Their conclusions are unanimous:
• No crude-oil residue survives the purification process.
• Toxicological testing shows no harm when consumed within the Acceptable Daily Intake (ADI).
• Food-grade dyes meet strict purity and safety standards before ever reaching store shelves.
The Bottom Line

When you see “Red 40” or “Yellow 5” on an ingredient label, you’re not eating crude oil—you’re consuming a single, well-characterized chemical that has been synthesized, purified, and tested for safety. The next time someone claims you’re “eating petroleum,” rest assured that modern chemistry and regulatory science stand firmly behind the safety of these widely used food colorings.

MASA STRONGLY CONDEMNS RFK JR. FOR HIS ABSURD RECOMMENDATION TO STOP USING FLUORIDE

His complete lack of qualifications and relentless rejection of science are both reckless and indefensible. A political favor got him that nomination.

RFK JR. is beyond reckless and uneducated, he MUST BE FIRED IMMEDIATELY ALONG WITH ALL THE MAHA MUPPETS.
RFK Jr. continues to reveal his staggering incompetence and disregard for science by promoting the reckless idea that states should eliminate fluoride from public water systems. This is not just ignorance — it’s a deliberate injection of his conspiracy theories into public policy. His anti-science agenda, rooted in lies/misinformation and fear, is actively undermining decades of proven public health measures. RFK Jr.’s crusade against fluoride is not based on fact, evidence, or medical expertise — it’s based on fringe beliefs that have no place in serious governance. His actions aren’t just irresponsible — they’re endangering lives and setting back the health of communities across the country. It’s a calculated effort to force his conspiracy theories into public policy, regardless of truth or consequence.

MASA is in the process of starting the STOP THE LIES CAMPAIGN directed at all the liars of MAHA like Vani Hari aka The FoodBabe, RFK Jr., who is grossly uneducated, who falsely claims there is no such thing as a good chemical, the first time I heard that, I went from a full head of hair to bald, like I am now. H2O. Conversation over, wow, just wow. This crazy fraud has a seat in the white house. Her fellow MAHA alum Casey & Calley Means are just vile liars. Calley was trying to make a point recently about John Rockefeller and his lawyer, Abraham Flexner. Calley is so uneducated, or he lied, I’m going with both, he thought Flexner was Rockefeller’s attorney, but no, Calley, he is not. He actually designed the amazing healthcare system we have today.

In recent months, the United States has faced a significant measles outbreak, with over 600 cases reported across 22 states as of early April 2025. Texas has been the epicenter, reporting 505 cases and two child fatalities, both among unvaccinated individuals. The Centers for Disease Control and Prevention (CDC) has highlighted that 97% of these cases involve individuals who were unvaccinated or had unknown vaccination status. CBS News+5news+5Axios+5ReutersCDC
What is RFK’s Response? Straight denial, reverting back to his anti-vaccine rhetoric, just vile. His old group, the Children’s Defense Fund, immediately claimed it was the vaccine, but even those scientists couldn’t get it right.

Health and Human Services Secretary Robert F. Kennedy Jr., known for his longstanding vaccine skepticism, has faced criticism for his handling of the outbreak. During a visit to the affected Mennonite community in Seminole, Texas, Kennedy offered condolences but did not advocate for the MMR (measles, mumps, and rubella) vaccine, the primary preventive measure against measles. Instead, he promoted unproven treatments, such as budesonide and clarithromycin, which lack scientific support for effectiveness against measles. New York Magazine+3The Indianapolis Star+3AP News+3news+1The Guardian+1

This approach has drawn deep concern from medical professionals and public health experts. The father of one of the deceased children expressed disappointment in Kennedy's silence on vaccination during his visit. Subsequently, Kennedy released a statement acknowledging the vaccine's efficacy but continued to endorse alternative treatments lacking empirical validation. The Guardian

The CDC and other health authorities emphasize that the MMR vaccine is safe and highly effective, providing immunity to over 97% of recipients after two doses. They continue to urge vaccination as the most reliable method to prevent measles and curb the current outbreak. AxiosLatest news & breaking headlines

RFK Jr.: A Dangerous Failure for Public Health and Truth
In his short time on the national stage as a candidate and now in public service, Robert F. Kennedy Jr. has demonstrated a dangerous mix of ignorance, arrogance, and disregard for science. His tenure has been marked not by progress or reform, but by a relentless campaign of misinformation, anti-scientific rhetoric, and personal vendettas against the very institutions that protect public health. RFK Jr. is not just wrong — he is actively harmful to the American people.

🧪 Anti-Vaccine Lies and Scientific Sabotage
RFK Jr. is perhaps best known for his unrelenting war against vaccines, a crusade he’s waged for over 20 years. He has falsely claimed that vaccines are linked to autism — a myth long debunked by the CDC, WHO, and over 25 peer-reviewed studies involving millions of children globally. His organization, Children’s Health Defense, has been identified by the Center for Countering Digital Hate as one of the largest sources of vaccine misinformation in the world.

During the COVID-19 pandemic, Kennedy spread deadly falsehoods, promoting unproven treatments like ivermectin and hydroxychloroquine even after clinical trials showed they were ineffective. He questioned the very existence of the virus’s threat and even floated the racist conspiracy that COVID-19 was engineered to spare specific ethnic groups. This claim was condemned by scientists, ethicists, and members of his own family.

For the anti-vaccine crowd, you are not smarter than a doctor or scientist. Not even you, RFK Jr., or his star pupil Vani Hari. The fact that he supports her clearly shows how inept he is. He cannot tell a scientist or uneducated charlatans scamming Americans.

Perhaps most alarmingly, since stepping into a public leadership role, he has fired experienced vaccine safety officials, including the FDA’s Dr. Peter Marks, who resigned in protest, citing Kennedy’s commitment to “misinformation and lies” over science.

💧 Fluoride Fearmongering
RFK Jr. has also targeted one of the most effective public health interventions in American history: water fluoridation. Calling it a “toxin” and “brain poison,” he has pledged to eliminate it nationwide, even though fluoride at regulated levels is safe and helps prevent cavities for millions of Americans, especially low-income children.

The CDC, ADA, WHO, and American Academy of Pediatrics all endorse community fluoridation as safe, cost-effective, and essential. Kennedy’s claims rely on out-of-context foreign studies conducted in places with excessively high natural fluoride, not the controlled, optimal levels used in the U.S. His push to ban fluoride is not just scientifically unsound — it's an act of cruelty that would disproportionately hurt the most vulnerable.

🥛 Promoting Raw Milk and Ignoring Food Safety
Kennedy has also begun championing raw (unpasteurized) milk, which the FDA warns can carry dangerous bacteria like Salmonella, E. coli, and Listeria. Children, the elderly, and pregnant women are particularly at risk — yet Kennedy, to push his “health freedom” agenda, has ignored decades of food safety science. This isn't reform. It's recklessness.
📉 Dismantling Public Health Infrastructure

Since taking office, Kennedy has gutted the Department of Health and Human Services, firing thousands of communications staff and scientific advisors. He has centralized control, limiting transparency and expert input in critical areas like vaccine safety, environmental health, and emergency response. The CDC has reported drops in childhood vaccination rates, and public health leaders warn we may see a resurgence of preventable diseases like measles and polio.

Kennedy has also mocked the idea that health leadership should require medical expertise, saying the problem isn’t lack of qualifications, but “corruption” — an excuse to replace scientists with loyalists who share his fringe views.

🔥 Conspiracy and Chaos: A Leadership Philosophy
At the heart of RFK Jr.’s platform is a deep distrust of government, science, and medicine. He sees plots in every public program, from dietary guidelines to FDA approvals. His speeches and writings are filled with baseless claims about “toxins,” “pharma cartels,” and “mass poisoning.” His best-selling book about Dr. Anthony Fauci was so riddled with falsehoods and misleading claims that it was publicly discredited by nearly every health expert who reviewed it.

Even his signature initiative, the Make America Healthy Again (MAHA) movement, is riddled with contradictions, lies, and misinformation. He rails against “toxins” while being photographed eating fast food with Donald Trump. He condemns processed food yet promotes meat-heavy diets against USDA recommendations. MAHA isn’t a health policy — it’s a performance. A populist brand built on fear, not facts.

Conclusion: Unfit, Untruthful, and Unqualified
Robert F. Kennedy Jr. is not a visionary. He is not a reformer. He is not a public health advocate. He is a man without qualifications, without integrity, and without respect for truth. His lies are not harmless — they erode trust, damage institutions, and put lives at risk.
At a time when public health needs clarity, leadership, and science, RFK Jr. offers only chaos, conspiracies, and confusion. The American people deserve better.

In recent months, the United States has faced a significant measles outbreak, with over 600 cases reported across 22 states as of early April 2025. Texas has been the epicenter, reporting 505 cases and two child fatalities, both among unvaccinated individuals. This approach has drawn concern from medical professionals and public health experts.

Here are clear examples of RFK Jr. and Vani Hari being thoroughly debunked by our own Dr. Idz. It’s disgraceful that the Secretary of Health and Human Services can’t even pronounce widely recognized names in the health and science community. A truly qualified leader would know them and understand the science behind them. They’re easy to learn.

MASA rigorously debunks every one of their falsehoods: dyes are safe, vaccines save millions, and raw milk carries real dangers.
