We recently came together as a team for our 2026 Kick-Off Meeting, which gave our incredible care team a chance to reset and reflect on what last year asked of us.

We can’t help but feel immense pride in our team and in our continued commitment to our partners, the families we serve, and the seniors we advocate for. At the heart of our mission is the responsibility to lead with integrity, excellence, and genuine care.

In 2026, we’re growing with purpose and continuing to raise the bar in senior care advocacy by pairing trusted placement expertise with added clinical insight. This allows us to support families with guidance that is both compassionate and clinically informed.

We’re excited to welcome Chantae Medina to the Gentle Hearts Living Solutions team! 🩷

With nearly two decades of experience in healthcare, Chantae brings deep clinical knowledge and a genuine heart for compassionate care. She holds a Bachelor of Science in Nursing and has a background spanning geriatric care, oncology, and hospice nursing. She is known for building strong, trusting relationships and for being a thoughtful advocate for the individuals and families she serves.

Chantae will be joining our work in the East Valley of Phoenix, supporting families, care homes, and partners. We’re so grateful to have you on the team, Chantae, and we’re so glad you’re here!

It was a beautiful, heartfelt evening that captured the dedication and thoughtfulness their team brings to those they serve every day. We’re so grateful to work alongside the incredible professionals at The Center at Arrowhead and to support the impact they make for seniors and families in our community. 🩷

We’re committed to supporting seniors and the people who care for them.

Every Tuesday from 9:30–11:30am at The American Lutheran Church of Sun City (17200 N. Del Webb Blvd), our own Kari McLeod leads a weekly caregiver support group designed specifically for loved ones and caregivers of individuals living with dementia.

Caring for someone with memory loss can be overwhelming emotionally, physically, and mentally. Many caregivers experience isolation, stress, or burnout on their journey. Support groups provide a safe, compassionate space where individuals can share experiences, gain perspective, and connect with others who truly understand.

This group focuses on:
• Emotional support and shared experience
• Self-care strategies for caregivers
• A welcoming, judgement-free environment
• Peer connection and community

There’s strength in connection, and caregivers shouldn’t walk this path alone. If you or someone you know is caring for a loved one with dementia, we invite you to join us – this isn't a journey you need to walk alone. 🩷

As we step into the new year, we want to express our sincere gratitude to the broader care community we have the privilege of working alongside.

Supporting seniors and families through complex transitions requires trust, collaboration, and informed decision-making. We are grateful for the confidence placed in Gentle Hearts Living Solutions to provide guidance that is both clinically informed and deeply practical.

Over the past year, we’ve continued to grow, strengthening our Registered Nurse-led approach, expanding our internal capabilities, and refining our processes to better support families navigating increasingly complex care needs. This growth has been shaped by collaboration and a shared commitment to doing what is right for each individual situation.

As we begin this business new year, our focus remains clear:

Today, we’re looking back on the journey our team shared throughout 2025, and we’re deeply grateful for the trust families and referral partners across Arizona and Idaho have placed in us. Every part of the senior care journey begins with heart. We’re inspired daily by the stories we hear and the relationships we’ve built—from team wins like Coffee Connections growing to over 40 new faces to celebrating partners at the Ensign Flag Award Ceremony and supporting local community events.

We know this work isn’t possible without the care homes and facilities we work alongside to find the right fit for each person’s unique medical, physical, and emotional needs. We are incredibly thankful for the commitment and partnership each of you brings to ensure our seniors receive expert, dignified care.

To every family who trusted our local advocates to be a source of clarity and compassion this year: thank you for letting us be part of your story. We are honored to continue making a difference in the lives of those we serve.

As the holiday season begins, we can't help but feel incredibly thankful for the relationships that make this work so special. Being surrounded by organizations like United Hospice, who show up with so much heart and care for seniors and their families, makes this work so meaningful.

This week, our Senior Placement & Referral Specialist, Zac, joined Aspira Arrowhead for a special Veterans Day ceremony honoring the veterans living within their community.

We were grateful for the opportunity to partner with United Hospice & Palliative Care of Arizona in recognizing each veteran with a commemorative pendant. Zac led the ceremony in prayer and honored each veteran for their service, their courage, and their sacrifices.

We’ve been so fortunate to meet so many incredible veterans and families over the years, and getting to honor them and express our appreciation meant so much to us.

We are deeply grateful to every veteran who has served our country, and incredibly proud of Zac for leading such a meaningful ceremony for this community! 🇺🇸

Today we’re honored to celebrate two incredible women who bring so much to our team and to every family we serve!

Heather, our Office Manager, is celebrating her first year with Gentle Hearts! We’re so grateful for the intention, structure, and heart she brings to everything she does. Her impact is so much more than the day-to-day. She’s the steady hand behind the scenes, equipping our team to show up for families and for one another.

Kari McLeod is celebrating four amazing years as a Senior Placement & Referral Specialist with Gentle Hearts! Known for her deep compassion and advocacy, Kari has guided countless families with empathy and expertise, always ensuring they feel understood and supported with the right resources. Her work reaches beyond placement as a trusted guide and advocate within the senior care community.

Thank you, Heather and Kari, for all you bring to our team every day. We’re so lucky to have you both and can’t wait to celebrate many more years together! 🩷

We had such an incredible morning at yesterday’s Coffee Connections in Scottsdale! 🩷

We’re so grateful to everyone who took time out of their day to join our Senior Placement and Referral Specialist Jonathan’s third Coffee Connections event. We loved seeing so many familiar faces and welcoming new ones — all gathered around the shared mission of serving our senior community.

It’s been incredible to watch this community grow and to see so many caring professionals come together to connect and support one another. Thank you to everyone who joined us and helped make it such a special morning!

If you couldn’t make it this time, we’d love to see you at our next Coffee Connections at Drip Coffee & Desserts in Glendale on November 20th at 9am. We hope to see you there!
