Heart failure is a major national health challenge. It remains the only cardiovascular disease that continues to rise, and it's the number one cause of hospitalization in adults over 65.

During Heart Month, we're highlighting our Heart Failure Empowerment Program designed for our home health patients. Compassionate Care Hospice held its Annual Memorial Service this evening for the families of all our patients who have passed in the last year. This was where families (and staff) gathered together in person and online to laugh, cry, reunite, but above all else –remember those that have gone before us. It is an honor and privilege for our staff to provide such loving care to those patients and families in our community. We hope the memories of your loved one bring comfort. 💚💚
