Tri-County Health Network is a non-profit organization committed to promoting health equity, removing barriers to care, and improving access to health services across rural communities in Southwest Colorado. We serve San Miguel, Ouray, Montrose and Delta counties.

Formed in 2008, we believe that health is more than healthcare and envision communities that are vibrant and healthy where everyone has the opportunity and ability to thrive.
