Live Well Physical Therapy, LLC is a cash based practice. This means I do not take insurance, however I can provide you a super bill (invoice) that you can self submit to your insurance. This business model allows me to provide the most needed services each visit with a reduction in total visits over a course of treatment. My name is Angie Huber, and I am the owner and Doctor of Physical Therapy provider for Live Well Physical Therapy, LLC. I am a Board-Certified Clinical Specialist in Orthopaedic Physical Therapy and have been seeing patients since January 2009. I have a passion for orthopaedics, vestibular rehabilitation, and helping others help themselves through wellness, prevention, and education. I am an Assistant Professor currently teaching at the University of Findlay. I teach courses related to physical therapy examination, medical screening, orthopaedics, and therapeutic exercise and present scholarship in the areas of health focused physical therapy care and prevention, intervention, and performance in the sport of running. I cannot wait to begin regularly providing physical therapy services to this wonderful community!
