Whether they’re advocating for services, walking a family through a care plan, or connecting clients to basic needs—CHWs are essential. At HCNO, we’re proud to support a regional network of CHWs through the Northwest Ohio Pathways HUB.

We are spreading maternal health awareness! A reminder that many still face preventable risks, disparities, and gaps in care.

At HCNO, we know that maternal health isn’t just about what happens in the delivery room. It’s about housing, transportation, food access, mental health support, and trusted relationships—before, during, and after birth.

Through our Northwest Ohio Pathways HUB, we work with a dedicated network of Community Health Workers (CHWs) who walk alongside mothers and families, helping them connect to care and advocate for themselves at every step.

Because every parent deserves a safe birth, a healthy baby, and a support system that sees the full picture. Learn more at https://www.hcno.org/nwopathwayshub

Last week some of the HCNO staff participated in a site visit at Salem Lutheran Church, one of the “Getting Healthy Zone” anchor organizations. Staff learned about the church’s programs and history while touring the wonderful facility. A highlight was viewing the kitchen and community gardens-- a part of the church’s community efforts. Check out the images below to learn more about the collaborative efforts between Salem Lutheran Church, the Northwest Ohio Pathways HUB, and the Getting Healthy Zone project.
