Lily Hospice was honored to participate in an evening dedicated to education, collaboration, and improving the way we support patients and families through important healthcare transitions.

We would also like to thank Martha T. Berry Medical Care Facility for graciously hosting and helping bring together such an engaged group of nurses, social workers, case managers, and healthcare professionals.

These conversations matter, and we are grateful to be part of a healthcare community committed to compassionate, patient-centered care.

Behind every service member or veteran navigating injury or illness, there is often a caregiver providing strength, compassion, and unwavering support. Their role is not always seen, but it is deeply felt.
