We’re looking for an Adult Functional Medicine Nurse Practitioner to join our growing team. This role is all about providing holistic, patient-centered care that helps adults heal, restore, and thrive.

📍 Location: Fully Remote OR Hybrid (Licensing in other states is accepted, we will assist in getting your licensure in IL) Hybrid role includes in-office care at our Crystal Lake, IL location.

✨ What you’ll do:
✔️ Work with adult patients (not pediatrics) to uncover and treat root causes
✔️ Use advanced testing (hormones, micronutrients, GI mapping, genetics & more)
✔️ Create individualized care plans that blend lifestyle, nutrition, supplementation & integrative therapies
✔️ Empower patients to take control of their health for the long-term
