Choices for Change Counseling Agency and WMed Street Medicine have entered into a new, collaborative mission that combines street medicine and case management, as well as addictions treatment, psychiatric interventions, crisis response, cognitive-behavioral therapy, and reality therapy. The clinic and medical organization will be jointly working with clients in their homes, motels, tents, or wherever the patients/clients are located.

The teams will encompass doctors, social workers, counselors, student doctors, interns, masters'-level counselors and social workers engaging in ACTIVE (not only reasonable) EFFORTS at the micro-level to connect with clients/patients to build trusting relationships.

This approach takes into account transportation barriers faced by so many in the community. We fill the transportation gaps and accompany clients/patients if so desire. We started outreach care this week. We are so excited about our new joint venture.

Mazel Tov to Choices For Change Counseling Agency on their 33rd anniversary. For 33 years Choices For Change staff has worked hard providing mental health treatment and case management in the office, on-line, in clients homes and in schools helping people to heal, address child abuse, violence, relationship difficulties, substance abuse and other arenas that challenge their lives. Everyday our staff helps to improve families, build closer marriages and better self esteem one person at a time.

We are the first private practice in this community that took Medicaid and provides therapy in the homes and schools and offers case management as a part if treatment even with the very low rates of reimbursement.

We have an exceptional staff which includes: Troy Zukowski-Serlin,LMSW, Tom Duffield, MA/LPC, Dennis LeClear, MA/LPC, Dr. Russell Hibbard, Connie Brooks,MA/LPC, Robert Washington, MA/LPC, Mya White, LLMSW, Michelle Zukowski-Serlin, LMSW, Roxana Almonacid-DeSinn, MA/TLLP, Office Manager Jane Fries, Biller Specialist Tracy O’Connor, and our masters level interns in WMU’s counseling program Abby Tignor and Symone Thomas.

This was an especially difficult year losing one of our valued staff Patricia Carlin, MA/LPC/LMSW to cancer. We are grateful to our clients who give us the gift everyday of being a part of their journey to heal and build more fulfilling lives. Thank you for your trust in us.
