Five Wishes is a user-friendly document and advance care planning program that allows individuals to express their wishes for medical treatment, personal care, and how they want to be treated at the end of life. It functions as a legal document, similar to a living will, but in simple terms, covering who should make decisions for them, the type of medical treatment they want, how comfortable they want to be, and what they want their loved ones to know.
