Wives, girlfriends, partners, this one is for you! For education and support.

I have treated thousands of men with prostate cancer and every one has his own story. And behind almost every one of these men is a partner trying to figure out what it all means for their relationship and how to help.

It's not an easy road but there is a light at the end of the tunnel. It brings some couples closer together. Unfortunately I have also seen relationships crumble under the hefty weight of prostate cancer treatment.

Yeah I'll take some testosterone and a large fry.

The McDonaldsification of men's health is not great. Sometimes I have to laugh because it's a little depressing. Guys deserve more.

A testosterone level and a PSA is not a workup for not feeling well.

Speaking of fast food and also of high school, growing up in California we all put the In-N-Out bumper stickers on our cars thinking we were really cool. We were not.
Sigh, simpler times.

I get it, "the news" is entertainment. Chronic diseases are boring. Chronic anything is boring. We like acute stuff, fresh stuff. Chronic diseases are quite literally yesterday's news.

But remember, for a lot of us, chronic diseases will be tomorrow's news, unless we make changes now.

Stay out of the news cycle! Learn how to identify your risk factors for chronic diseases and mitigate them sooner rather than later. Do it for yourself and for those who need you.

Hey guys before your doctor just put you on ED medication or testosterone, make sure they discuss the potentially life altering underlying causes that may be lurking. Erections are great, I'm a big fan. But after a heart attack or stroke they may be less important. Don't let that happen.

If you're feeling poorly or having erectile dysfunction just having a testosterone checked is not a work up. And it is not "men's health." Ask for more.

So why does such a ridiculous boondoggle of codes even exist?

Spoiler, it's not to make patient care better and it's not to make doctors more efficient. Like much of the other administrative box clicking that doctors do, these codes exist for two reasons.

The first reason is so that insurance companies know exactly what we are doing and why. We have to successfully match the diagnosis code with the proper treatment code. This is sufficiently complex such that we mess up on occasion, which gives insurance companies the justification to decline payment. That's where the team of office staff necessary to deal with insurance companies comes in.

The second reason codes exist is so that data can be gathered about patient diagnoses and treatments. This data is very valuable to the likes of pharmaceutical companies, insurance companies, etc. and as often mined and sold.

If you read through a corporate medicine doctor's notes it is sometimes difficult to know exactly what the patient complaint was or what the plan is, but instead you will see several pages of codes that exist for the purpose of getting adequstely reimbursed.

It's so refreshing at The Men's Center since we have 86'd the middlemen. Instead of codes in our notes we actually have a narrative that explains exactly what the patient's complaints are and what we are doing about it. I have not entered a code in a note in three years. It is glorious.

Furthermore, the fact that we don't spend over half of our time doing administrative tasks to satisfy insurance companies and Medicare, we are actually able to focus time on taking care of patients. This is why we don't have 10 minute visits and this is why we are available to answer phones and return emails.

And because there are no confusing bills that come two or three weeks after an encounter, patients get total price transparency, which gives them the power to actually act as consumers to make good informed healthcare decisions.
The doctor-patient relationship, brought back from the dead. We love it.

Sleep is just so important for longevity. But a lot of us, myself included, have not made it enough of a priority.

I have struggled with getting enough sleep and with sleep quality, and have found these supplements helpful.

Ashwagandha is another name for Indian ginseng or winter cherry. It is a small evergreen shrub native to India. It is often referred to as an "adaptogen" due to its possible benefit of decreasing cortisol and combating the effects of stress. There's also some limited evidence that it may improve s*xual health and fertility. Randomized, controlled trials and meta-analysis have suggested that Ashwagandha can improve sleep quality as well as total sleep time compared to placebo. Short-term side effects appear to be minimal and are primarily related to gastrointestinal side effects. Long-term safety studies are scant. It is not recommended during pregnancy.

Although high-quality randomized, controlled trials looking at magnesium for sleep are limited, correlation has definitely been reported between magnesium supplementation and sleep quality. The potential benefit of magnesium for sleep may be related to increasing GABA (a neurotransmitter that promotes relaxation) and supporting melatonin production. Magnesium is available in multiple forms but the potential advantage of magnesium-L-threonate is related to enhanced brain bioavailability. This supplement may also help improve cognitive function. Side effects of magnesium can include diarrhea.

Phosphatidylserine is a phospholipid that exists at high levels in brain cell membranes. It has been demonstrated in some studies to improve sleep disturbances related to chronic stress and may work by reducing cortisol levels. Evidence suggests that it may also improve memory and cognitive function although the studies are limited. It can be found in foods such as fish and sunflower. It is generally felt to be safe.

Please be sure to discuss any supplements that you take with your doctor prior to starting. We would be happy to discuss other strategies for sleep. Come on by and see us.

Yes I know it seems like a lot but you can do it! Longevity isn't just about living to be 100 it's about being able to do the things that you enjoy doing well into the second half of life. I don't care if it's pickle ball or cooking or crossword puzzles or having s*x. Or all of the above. Set yourself up to continue to be able to do the things that bring you joy. Changes you make now will improve your chances of having true healthspan. It's never too early and it's never too late.

Baby steps and watch the snowball start rolling in the right direction!
