It has been shown that the mandibular alveolar segment can be stimulated to grow in transverse dimension during early mixed dentition*. It is also known that facial growth occurs at different times with different rates. The growth in width is completed first, followed by depth and height. As the growth in width is waning at about seven years of age, growth in depth and height are accelerating. Given the tremendous amount of vertical growth still available during mixed dentition, any minor expansion of mandibular primary dentition projects the alveolar bone in a vertically expanding arc. Consequently, the initially expanded portion remodels into the apical base of the developing permanentdentition, i.e., the premolars and canines.

We established our company Mixed Dentition Orthodontics in 2019 primarily to promote the concept of avoiding extraction of teeth through growth modification in early to mid-mixed dentition phase of dental development. In principal, extraction of teeth becomes necessary when there is not enough space in the alveolar bone to accommodate permanent teeth. It is known by the time all permanent teeth erupt, most of facial growth in width and depth has been completed. Obviously, it is not possible to modify what is not available anymore. The mandibular arch in particular presents a challenge, since unlike the maxilla there is no sutural accommodation and expansion of the inter-canine width has proven unsuccessful.

Introducing out anatomically contoured expansion (ACE) screw, a mini-expansion screw that follows the contour of the palate and tongue in every dimension. The maxillary skeletal expander is designed to minimize the chances of palatal impingement. It is also much more comfortable to the patient during speech and swallowing than the standard rectangular-shaped expander.
