We’re excited to share that our practice model has been selected to participate in the CMS Innovation Center’s GUIDE model! This will allow us an opportunity to expand our collaboration with our participating network pharmacies to enhance the support of dementia care with their patients in their communities. Our team at PPC @ Home will train you and your staff on how to identify the ideal patient for this program. Patients who have fallen or are at high risk, those who are at the beginning of cognitive decline, and those who have recently been hospitalized are all examples of who you may already be working with and can identify through your pharmacy practice.

Today our team had a meeting with Dr. Lewis MD (Physiatry-Pharmacy Collaborative) to launch our new interprofessional patient care. We teamed up to help frail patients get stronger at home and improve their physical, emotional and cognitive health:) Thank you to CPESN PPC at Home team for facilitating this program!!! We can’t wait to bring some meaningful change to our patients’ quality of life with this new approach 💙
