All hail, community health workers! They are the connectors—bridging gaps between health care systems and the people who need them most.

At Constellation Quality Health, we built Vega to support their work. With intuitive tracking tools, data exports, and real-time outcomes metrics, Vega helps community health teams prove what they’ve always known: their programs save lives, reduce disparities, and create healthier communities.

When we give CHWs the right tools, we amplify their impact.

"I've worked at the Community Care Clinic for 10 years," said 2024 Harris Award recipient Peter Robin, M.D., who designated Community Care Clinic as his charity of choice. "The clinic provides care to the uninsured poor who have nowhere else to go for their health needs. It's a comprehensive care center staffed by a large volunteer group of providers. I'm proud to be affiliated with such a wonderful organization!"
