Community Health Center of Snohomish County (CHC) is a non-profit provider, founded in 1983 to serve the health needs of community members who face barriers to healthcare. We offer affordable primary healthcare services, including medical, medical walk-in, dental, physical therapy, and pharmacy at several locations throughout Snohomish County. In addition to our standard clinics, we operate school

-based health centers in local schools and run outreach programs in partnership with other local non-profits. Medical, Dental, Pharmacy, Behavioral Health and Nutrition

Community Health Center of Snohomish County (CHC) provides affordable health services including medical, dental, behavioral health, substance abuse treatment, and pharmacy services. We welcome patients who are on Washington Apple Health, Medicaid, Qualified Health Plans (QHPs), Medicare, Medicare Advantage, Tricare, select private insurances and those who are uninsured. For those without insurance, CHC will help patients sign up for Washington Apple Health; if ineligible, we offer a sliding fee discount based on household income and family size.

At CHC we believe in practicing patient-centered care, where our team of health care professionals work WITH you to build a relationship based on your health care needs.
