As we mark this milestone, we're excited to launch our Building Hope, Expanding Care capital campaign to fund major facility improvements including the build out of our new Port Jefferson Station location and complete renovation of our Riverhead facility. Meet Josephine Pizzurro, our Director of Population Health! 👩‍⚕️ With nearly 30 years in healthcare, she leads our dedicated team in facilitating care from the outside, in.

🔍 DID YOU KNOW our Population Health team:

1️⃣ Provides monthly outreach to patients in group homes and residential facilities - Our care managers connect with Medicare patients to assist with prescription refills, appointment scheduling, and serve as an extra set of eyes and ears for both patients and providers.

2️⃣ Operates with a comprehensive 16-member team - Including 2 virtual care registered nurses, 8 LPNs serving as care managers, 4 community health workers, and 2 medical assistants working together to coordinate seamless care.

3️⃣ Bridges the gap between patients and provider* - We collaborate directly with healthcare teams to identify and address issues before they become bigger problems, ensuring continuity of care for our most vulnerable community members.

This week (August 3-9), we join health centers across America in celebrating 60 years of improving our nation's health! As a proud Federally Qualified Health Center (FQHC), Long Island Select Healthcare embodies the PAST, PRESENT, and FUTURE of community-centered care.
