The Supported Living Specialist provides and/or coordinates supports that are necessary to enable Complete Care Supported Living Agency consumers to live as independently as possible. This includes assisting in the planning, implementing and documenting of the My Plan’s, insuring the well-being, safety and quality of life of program consumers, providing guidance and training that facilitates independence and recognizing and respecting the rights of the consumer.
DUTIES AND RESPONSIBILITIES:
1. Protect, advocate for and insure all rights of consumers participating in Complete Care Supported Living Agency programs.
2. Contribute to the ongoing planning of services to include the preparation of My Plan’s and the attendance of consumer staffing.
3. Implement training programs specified by consumers My Plan’s
4. Assist consumers in making decisions that will result in their being viewed by others as valued members of the community. This includes but is not limited to personal appearance.
5. Assist in the purchasing of clothing for assigned consumers.
6. Implement services that are built around the consumer’s wants and needs, are culturally and age appropriate, and aim toward maximizing community participation.
7. Monitor and coordinate consumer health needs to include routine and emergency medical appointments, medication acquisition and monitoring.
8. Assist with the personal hygiene as needed by the consumers to include but not limited to bathing, toileting and dressing.
9. Provide transportation to and from work, recreational activities, appointments, etc., as specified in the Residential Procedures.
10. Monitor building maintenance and performs maintenance of living areas in accordance with rental contracts and/or Residential Procedures.
11. Prepare and maintain consumer budgets, financial records, and personal cash as required by the Residential Procedures.
12. Monitors temperature, blood pressure, weight, mental status, intake/output, medical orders, hydration, and general health of consumers.
13. Purchase program equipment and supplies in accordance with Residential Procedures.
14. Coordinate program activities with other Complete Care Supported Living Agency programs and area agencies.
15. Maintain up-to-date familiarity with state laws, regulations and legislation and assure compliance.
16. Attend training seminars, in-service trainings and staff meetings. Participate as required, in development and implementation of training etc.
17. Assist with meal preparation as required ensuring adequate nutritional needs are being met for the consumer.
18. Maintain professional working relationship with co-workers and key personnel.
19. Develop and maintain a positive, supportive rapport with consumers.
20. Complete consumer program records, documentation, and reports in accordance with Complete Care Supported Living Agency Policy and Procedures and Residential Procedures as required for D.O.D.D. and, Medicaid Waivers, etc.
21. Administers Medication and Maintain Medication Logs.
MINIMUM EDUCATION, EXPERIENCE, and SKILL REQUIREMENTS:
1. A high school diploma or equivalent.
2. A minimum of one-year experience with adults who have intellectual and/or developmental disabilities (or similar experience) is strongly preferred.
3. Eight-hour New Provider Training (MUI Training).
4. First Aid, CPR, and Medication Administration Certification.
5. Bureau of Criminal Identification and Investigation (BCI) or FBI checks.
6. Position requires a valid Ohio driver’s license, an acceptable driving record, and a registered and insured vehicle.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
1. Demonstrate a personal commitment to the development and implementation of the least restrictive residential services for consumers.
2. Demonstrate skill in implementing and documenting direct consumer training.
3. The administrative and management skills necessary to supervise the day-to-day activities of residential consumers.
4. The ability to communicate effectively with other staff, consumers, and the general public. Flexible scheduling necessary
WORKING CONDITIONS:
1. Medium Work: Exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
