Supporting Community Connections, or SCC, is a suicide prevention outreach program for transitional aged youth (TAY) in the foster, homeless, and LGBTQI communities. This program seeks to prevent suicide by offering connections and linkages to housing opportunities, mental health services, women’s health providers, LGBTQI resources, and other social services in Sacramento County. Our goal is to he

lp youth feel empowered, healthy and connected! This program is funded by the Sacramento County Division of Behavioral Health Services through the voter approved Proposition 63, Mental Health Services Act (MHSA)
