This patient is approximately one month postop from . This is the perfect solution for patients who have that frustrating darkness and “tear trough” that is only obvious in pictures and at certain angles. It is a procedure: An incision is made inside the eyelid and I that annoying indentation from the INSIDE, where there is basically a ligament tethering the skin through the deeper tissue to the bone. The fat is then draped across the area that was hollow and smoothes out the eyelid/cheek transition.

I LOVE this procedure, and I’ve learned that it makes a huge difference even in a patient like this that almost doesn’t need anything.

These patients have often had unsuccessful in the past.

I am grateful that this procedure was embraced and taught during my training and have to give a thank you to for pioneering and teaching his technique. 😊

Please excuse the shadows and unprofessional background in the afters—I had to go across the pond to get these 11 YEAR postops. But while the pics are not perfect they do demonstrate an important point in : the main difference between implants placed OVER or UNDER the muscle.

When this 56 year old woman came to me she had 20 year-old implants OVER the muscle with:

*Baker III capsular contracture creating that “two balls on the chest” look and they were rock hard.

*Sagging of her breast footprint on the chest wall

The procedure I performed was:

silicone and replacement with new ones “under the muscle” or through her pre-existing incisions

11 YEARS LATER they remain soft and much more natural-looking because she is thin with very little breast tissue and the upper poles of the implants are blunted with the muscle coverage. AND they are actually less saggy than the others when she was YOUNGER.

It is important for patients to understand that while there is a small risk of animation deformity with an implant at least even partially under the muscle, when someone has a thin chest wall with small breasts to begin with, in my opinion placing them or “over the muscle” actually looks less “natural.”
In addition, historically most breast implants have a lower rate of capsular contracture under the muscle vs over.

In summary:

It is counterintuitive but generally breast implants look and feel more natural with a lower capsular contracture rate when placed under the muscle.

Fat grafting CAN help camouflage the top border of the implant but it is not completely reliable.

Personally, I would almost always recommend the or approach.

Honestly, animation deformity in cosmetic breast augmentation is rarely a complaint and is more common in breast reconstruction.

I would LOVE to get her in for a with my for an extensive face neck and next time she is in town and has time. We did a limited one on her central face during the surgery but you really should wait six months after a facelift to address the skin that’s been lifted. She admits she’s had a lot of sun damage when she was younger and although she’s been careful with the in her adult life…growing up in in the days of baby oil and tin foil catch up with you. 😬Not her fault. We didn’t know back then 😭

Our lovely patient is now six weeks out from her and under . This skin-only browlift is a great option for someone with very droopy lateral brows who doesn’t have much downtime because it’s an easy few days of recovery. And in fact did you know that you can actually get THE MOST LIFT of the brow this way? Our 55 year-old patient is three weeks after a lower with to the neck and and to her chin, and and and now just looks like a 🥰with a soft and natural result

A special Renuvion handpiece was used for the under eye because she did not want to do a
(I’m still working on her for that!)

All in all, she and I are both thrilled with her that has taken at least fifteen years off her face and left her with a soft natural glow.

The fat transfer really helped build up volume and definition in her jawline.

But the structural changes are only part of the transformation. and are the other big pieces of the puzzle. It all goes hand-in-hand.

Throwing it ALL THE WAY BACK to my training In #2003 😱when I did a on a 13 year old patient who was living at a care center for abused teens. I always treasure these and I always remembered how she had carefully alternated colors for the different letters.

And people need to know that while the tissue may grow back and it may need to be done again, there is no reason to make a young woman with severe symptomatic wait to have a reduction. plasticsurgery 🩷

A yet for our patient! This ( )under is one of my favorite procedures to improve with 🥰For this patient I also removed a tiny bit of fat under the chin. This fat cannot be removed with . Then the platysma muscle is tightened together, getting rid of those and up that to give you that look. Basically, it’s a 😉
