Foundations Health For patients struggling with opioid use, our outpatient center fosters a culture of healing and community to help with your process of health and wellness.

This program provides medication-assisted treatment (MAT) in addition to counseling services. Foundations Health fosters a culture of healing and community to help with your process of health and wellness throughout substance use disorders, mental health issues and opioid dependence. Foundations Health is proud to be a member of Foundations Recovery Network. Physicians are on the medical staff of Foundations Health, but, with limited exceptions, are independent practitioners who are not employees or agents of Foundations Health. The facility shall not be liable for actions or treatments provided by physicians. Model representations of real patients are shown. We are unable to provide medical advice through social media.
