Crossroad Health Center Crossroad Health Center offers comprehensive primary health care to children and adults. Crossroad is a proud partner agency of United Way.

Locations in Over-the-Rhine, Western Hills, Harrison, Rothenberg Preparatory Academy and Taft Elementary School. Crossroad Health Center opened in 1992 with a mission to provide accessible, affordable, community-based primary health care services and mental health counseling to the residents of Over-the-Rhine and the surrounding Cincinnati communities. In 2009, Crossroad expanded services to the W

est Side by opening Crossroad Health Center West. In 2014, Crossroad expanded again by opening a School-Based Health Center in Rothenberg Preparatory Academy and Crossroad Health Center Harrison. Emphasizing individual dignity and preventive care, Crossroad has become an increasingly important community resource, serving nearly 13,000 children and adults who otherwise would not be able to afford care.

🎆 Holiday Closing Announcement 🎆

Crossroad Health Center will be closed on Friday, July 4th in observance of Independence Day, as well as on Saturday, July 5th.

While our offices will be closed, on-call services remain available for urgent medical needs or emergencies. If you require immediate assistance, please call your clinic’s regular phone number to be connected to our on-call provider.

We wish everyone a safe, healthy, and joyful holiday weekend as we celebrate our nation's independence!

On June 25, Crossroad, with partners 3CDC, Truist & the Corporation for Supportive Housing, broke ground for our new OTR location near Findlay Market. Mayor Aftab Pureval was present & recognized the city's support for Crossroad & our mission. The new site is expected to open in April 2026.

Crossroad Health Center opened in 1992 with a mission to provide accessible, affordable, community-based primary health care services and behavioral health counseling to the residents of Over-the-Rhine and the surrounding Cincinnati communities. In 2009, Crossroad expanded services to the West Side by opening Crossroad Health Center West. In 2014, Crossroad expanded again by opening a School-Based Health Center in Rothenberg Preparatory Academy and Crossroad Health Center Harrison. Crossroad added another school-based health center at Taft Elementary School. This site serves the children and families of Taft, as well as community residents. Emphasizing individual dignity and preventive care, Crossroad has become an increasingly important community resource, serving nearly 13,000 children and adults who otherwise would not be able to afford care. Crossroad is a proud partner agency of United Way.
