Our Story

Assistance for Life Care Management Services provides services for older adults and persons with disabilities in Wilmington and the surrounding areas. Care management includes personal consultation, coordination and advocacy for individuals and their families as they navigate the transitions and decisions inherent to aging.

Assistance for Life care managers complete comprehensive assessments encompassing the following areas: Health Status & History, Living Situation, Support Network, Financial Status & Planning, and Legal Documentation. The care manager can then provide recommendations for local resources based on the client and families need.

On-going care management services are available as a means to ensure advocacy and consistent communication with our clients, their families, physicians and other professionals along the continuum of care. Individuals and families, especially those that live out of the area, will have peace of mind that they have an advocate is by their side during a transition or should a crisis arise. Care managers are available 24/7.

The goal at Assistance for Life is to assess client needs and wishes and then provide guidance and recommendations that will enhance or ensure the best quality of life.
