Case of the week #4: CarboTrack in Prehospital Management of Status Epilepticus
A 49-year-old male in status epilepticus required immediate airway management. EMTs suctioned the airway, inserted a nasopharyngeal airway, and administered high-flow oxygen (15 L/min).
Using CarboTrack, EMTs continuously monitored ETCO₂, gaining real-time insight into ventilatory status—crucial for guiding safe and effective care during transport.

GI Case Study of the Week #2: A 60-year-old woman undergoing ERCP for common duct stones with stent placement. Anesthesia included TIVA with propofol, midazolam, and fentanyl. Oxygen supplied via Salter nasal cannula at 3L/min with CO2 monitoring. Initially, capnography tracing was normal, but as sedation deepened, the patient began mouth breathing. Despite good oxygen saturation, capnography tracing was lost. Applying a device directly onto a side opening of the mouth guard restored strong CO2 capture and capnography tracing.

During sedation, airway obstruction occurred; placed OPA and switched to oxygen face mask at 8L/min, boosting saturation to 99%. Added CarboTrack™ for robust capnography, confirming good ventilation. Procedure concluded without further issues.

A recently closed claim from The Crypt 2023 has shaken the medical community. Tragically, a patient lost their life in the ER following a basic colonoscopy procedure, where continuous end-tidal carbon dioxide (ETCO2) monitoring was neglected. This serves as a wake-up call for healthcare facilities to invest in and practitioners to prioritize appropriate monitoring, with a strong emphasis on ETCO2.

The McMurray Enhanced Airway (MEA) and CarboTrack can be used simultaneously in any deep sedation case where the patient develops upper airway obstruction (UAO).

Most anesthesia providers would start with nasal cannulas and commence sedation. Patients often develop UAO under deep sedation requiring a distal pharyngeal airway (DPA). Once the MEA is placed the CarboTrack device can be applied to the fl**ge of the airway for both oxygen supplementation and capnography.
