Cervical laminoplasty is a surgical procedure designed to relieve pressure on the spinal cord in the neck (cervical spine). This surgery is also performed through the back of the neck and is intended to retain the natural movement of the neck after surgery. The surgery involves great proficiency and the use of a microscope. Within a laminoplasty surgery, a small portion of the bony part of the spinal canal (lamina) is removed to enlarge the spinal canal, thereby relieving symptoms of nerve root compression. By enlarging the spinal canal, it can relieve symptoms of radiating pain or weakness or numbness in the arm or hand. The difference between a laminectomy and a laminoplasty is that more of the lamina is left in place in a laminoplasty procedure. Also in a laminoplasty, the trained neck surgeon leaves more of the normal bone and ligaments intact, and provides a protective layer that lessens scar tissue formation on the dural sac after surgery. “The benefit of these two surgeries is that they are minimally invasive, non-fusion, and motion preserving approaches that provide a faster return to activity,” adds Dr. Jatana. Dr. Jatana also provides other specialized neck surgeries that aren’t typically provided by other Colorado spine surgeons. Unlike many neck surgeries that involve fusion, where two vertebrae are locked together, the cervical Laminoforaminotomy procedure avoids fusing the neck vertebrae. This retains the natural movement in the neck. While “motion preservation” is the goal of most surgeries, most spine surgeons are not able to do a Laminoforaminotomy because of the experience needed. “Because most spine surgeons divide their time between neck surgery and low back surgery, they don’t gain the proficiency in this particular neck surgery,” explains Dr. Jatana. “Also, this neck surgery requires use of a microscope and the use of small 1mm and 2mm instruments to repair the damaged disc rather than removing all of the disc and replacing it with a fusion and metal instrumentation." New research studies by the North American Spine Society — the most respected international society of spine surgeons — have shown that when a person has a herniated disc in the neck, and traditional fusion surgery is performed, there is an increased incidence of herniation at an adjacent segment. For these people, the newest FDA-approved artificial discs for the cervical spine can be of great benefit, explains Sanjay Jatana, MD, a Colorado fellowship-trained orthopedic spine surgeon who specializes exclusively in neck surgery. “The downside of spinal fusion surgery is that it locks two vertebrae in place,” Dr. Jatana notes. “This in turn puts additional stress on the discs above and below the affected area, which can lead to further disc herniation above and below the damaged disc. This process is called “adjacent segment disease” and it’s one of the main issues artificial discs were designed to address.” Dr. Sanjay Jatana notes that the Mobi-C disc was the first disc approved by the US Food and Drug Administration (FDA) for use at two levels in the neck. This can be of great benefit to those people with degenerative discs at more than one level in the neck and would otherwise need a fusion at the other level which in turn would restrict rotation. The benefit of an artificial disc is that it retains the natural rotation of the vertebrae in the neck, which would otherwise be locked together in a traditional spine fusion surgery. By preserving motion, this lessens the risk that other adjacent discs might herniate as well. Dr. Jatana is trained in the use of a variety of artificial discs for the neck.

Philosophically, Dr. Jatana emphasizes non-surgical treatment options in advance of neck surgery. Nonsurgical treatment of herniated discs may require physical therapy, stretching, and muscle-strengthening exercises to help heal the affected area and prevent future injury. Additionally, heat and ice can reduce pain and inflammation. Non-surgical treatment options are usually enough to remedy pain and mobility issues associated with herniated discs. In rare cases, surgery may be necessary to remove the protruding portion of the disk and relieve the pain caused by the compressed nerve. If surgery is necessary, ongoing physical therapy will almost certainly be recommended.

Discs are located in the neck region (cervical), upper back (thoracic) and lower back (lumbar). Most disc herniations occur due to repetitive injuries, genetics, and normal aging. Only one-third of disc herniations are due to a particular injury. When the disc herniation presses against a spinal nerve or the spinal cord, the compression of spinal nerve roots can cause numbness and weakness of the affected muscles along the pathway of the pinched nerve.
