If you live in Riley County, please take the Community Needs Assessment survey using this link: https://tinyurl.com/Riley-Co-survey

This survey helps organizations like Konza and many others understand the health and social challenges of the communities we serve to identify unmet needs.

Your responses ensure our plans are grounded in local priorities for health and well-being.

All responses are confidential. The survey is open through November 30th.

Printed copies are available at our Manhattan clinic.

Please encourage your friends, neighbors, community contacts, and other residents of Riley County to take the survey so it will include responses from a wide range of people.

The 2025 Riley Co CNA is led by the Flint Hills Wellness Coalition and sponsor organizations include:

Flint Hills Wellness Coalition is happy to announce the survey link for the 2025 Community Needs Assessment for Riley County is now open.

The purposes of this survey are: 1) to get your input on the quality of life in the community and 2) to identify the unmet needs in the community.

Some questions ask you about your personal experiences in the community and some ask for your opinions about the community in general.

Anyone who lives in Riley County who is at least 18 years of age is invited to take the survey.

The survey takes about 20-minutes to complete. You can start and stop the survey if you stay on the same device. Please try to complete the survey as incomplete surveys are not as useful as completed ones.

Questions cover topics associated with individual health and community health such as housing, transportation, nutrition, public education, older adults, youth, economic well-being, mental health, and physical health.

Participation in the survey is voluntary, and your responses will be anonymous. While your participation is voluntary and confidential, all community input is valued.

Please scan the QR code in the image or click on the link in the comment section.

The survey will be open through Sunday, November 30.

Get kids involved in meal preparation! 🥦🍅 Let them pick out fruits and veggies at the grocery store or farmers market, and then have them help wash, peel, and chop them for meals.

When kids are involved in food preparation, it can encourage them to try new foods and learn where our foods come from 👩‍🌾 And with WIC, you can get fresh, canned, or frozen fruits and veggies to accommodate your families specific needs and preferences! 🙌

Our Story

About KPCHC: Find all the care you need, under one roof! Konza Prairie Community Health Center and Dental Center (Konza) is a not-for-profit charitable organization and one of several hundred Community Health Centers partially funded by the federal government. One of the chief objectives of Konza is to create a place where people want to come for their health care- removing barriers that hinder clients from obtaining the services they need. Konza services are available to all, regardless of race, creed, national origin, language, gender, or financial status. Because Konza receives federal financial support, qualifying patients are offered a sliding fee scale. The sliding scale makes health care affordable to people with or without insurance. Konza’s integrated support system gives patients the complete network of care needed for health. Clients can access primary and preventive health care, dental services and mental health and substance abuse treatment provided in a compassionate, respectful, and professional manner. Convenient, routine, and same-day appointments make accessing care easier. Services include laboratory, radiology, and an immunization clinic onsite so patients don’t have to travel for routine lab work, x-rays, or vaccines. Konza facilities are staffed with high-quality dentists, physicians, mid-levels, and nurses, all of whom work together to offer patients the best care possible. Konza strives to help patients develop lasting relationships with providers, ensuring continuous, quality care. Our History: In 1994, Konza Prairie Community Health Center was formed to meet the unmet health care needs of the Junction City, Geary County, and surrounding areas. Konza is governed by a Board of Directors made up of local individuals, a majority of which are Konza clients. Konza offers medical and dental services in both Junction City, KS and Manhattan, Kansas campuses, serving patients living in a five-county radius and continuously expanding services to more efficiently serve the greater Flint Hills region. Payments, Insurance, and Medication: Konza accepts most insurance plans including, but not limited to Blue Cross Blue Shield, Met Life, Medicaid, Medicare and Tri-Care. Konza also accepts clients with no insurance, ensuring access to care for all. Konza has an on-site Affordable Care Act Navigator to assist the community with enrolling in health care for themselves and their families. Konza also has multiple staff to assist patients in starting care at Konza and determining if they qualify for programs like KanCare. As part of the Low Cost Prescription Program, Konza is partnered with Walgreens, Walmart, and Kroger. Konza also provides a Medication Assistance Program in which the pharmacy works closely with patients, providers, and prescription drug manufacturers to assist patients in receiving lower-cost of free medication.
