By adopting this approach, our community health workers can engage three times more effectively than if we were solely supporting an individual. This inclusive strategy ensures that the entire family receives the necessary care and support, amplifying our impact within the community.

For families and consumers, accessing healthcare resources can be insurmountable at times. For families under multiple health plans engaging with health care can often be met with complexity, confusion, and distrust. As a result, health plans struggle to engage with beyond 37% of their members independently. However, through partnership with LocalConnections for Health, health plan engagement is significantly enhanced to reach up to 80% of members.

How do we achieve this?

Our approach involves leveraging trained and certified community health workers who act as trusted guides to healthcare resources and social support. Our health workers connect to families in their communities through community-based organizations, telephone services, and face-to-face interactions.

Our focus extends beyond setting individual health goals; we meticulously devise workflows and implement tailored next-best actions based on each family member’s specific health needs. This approach ensures that health plan members and families receive personalized guidance, placing them on a path towards a brighter and healthier future.

This rebrand is more than just a name change; it marks the beginning of a new era filled with expanded offerings and opportunities tailored to better serve our partners’ consumers and employees.

As we embark on a journey to deliver even more support to families and employees in their local communities, we wanted to highlight a couple of unique changes to how we are focusing our efforts moving forward.

We were thrilled by the fantastic attendance at the Men’s Health Awareness Program in Santa Ana, California!

Our attendees had the opportunity to listen to experts discuss topics such as cancer, mental health, and promoting healthy lifestyles for men. The morning was filled with not only informative conversations and valuable resources but also opportunities to connect with new members of the community.
