The forensic psychology team had conducted an in-depth examination to determine the cause of death of Deputy Expert Diplomat Arya Daru Pangayunan, who was found dead with his face wrapped in yellow duct tape at the Guest House Gondia in Central Jakarta on July 8. She was one of four suicidal patients endangered by the Spokane hospital, as discovered by the Washington Department of Health. The hospital repeatedly failed to follow safety standards and its own policies for screening and supervising suicidal patients. In Sarah's case, there was a delayed response when she left the pediatrics floor, went to a parking garage, and jumped from the fourth floor.
