Documenting Care at Home - DCAH Valerie Falzo is a RN who retired to take care of her friend who had cancer.

She developed "Documenting Care at Home" to teach people how to document care being given in the home & track other extremely important information using pre-designed documents.

I Hope that this finds everyone well. One of the people who took this course at the Trinity Church in Watervliet when I offered it at an earlier time has written a testimonial about it and I would like to share it with all of you. It speaks to the important point that a serious illness most often is a deceitful, unjust enemy that will blindside someone and that person's family without shame or remorse, wreaking havoc and chaos in its wake.

Back, I believe, in early 2024, Ms. Falzo gave her course, “Documenting Care at Home”, to members of our church. Supporting Ms. Falzo’s efforts, I took the course, not thinking that I would ever need the information. It was interesting and informative. I came home and put the course binders on my bookshelf, not thinking I would ever need it.
Twenty-seven years ago, my wife developed cancer. Luckily, a “lumpectomy”, and some radiation took care of it. Every year since she has had a mammogram. Luckily, they all came out cancer free….until last year. Last September (2024) they saw a shadow that they didn’t like. The cancer came back in the same spot.
After counseling with three different doctors, they all concurred that major surgery was called for. In January of 2025, she had her surgery. We were told to report to the hospital at 6:30 AM…. By 1:15 PM they put her in my car and told me to take her home. I was given a stack of papers with instructions.
We got home; I put her to bed and started going through the after-care instructions. She had tubes coming out of her side, hanging with plastic containers on the ends. A pinkish liquid was flowing out of the incision, into the bags. I was expected to empty the bags periodically, and measure how much liquid was collected, and keep a log of the amounts that drained each day! I knew nothing about such things, nor had any training! Then there were the medications that had to be administered at different times during the day. Then there were the appointments to see the surgeon, radiologist, and the oncologist. Three different doctors at three different locations.
I was sitting at my kitchen table thinking that I may be having a panic attack! Then I remembered Ms. Falzo’s course and wondering if anything in it could be applied to my current situation…. if only I could remember where I put the course binder! Another panic attack, and I finally found it! I could feel my panic level subsiding!
The sheets she provides for keeping records were invaluable. The medication log made it easy to keep track of all her old and new medications. I recorded every doctor’s visit with vital information on a doctor’s visit log. And there were many. There was a total of five different doctors I had to keep track of. There was also a sheet that I kept the name and contact information for each doctor. On another sheet, I kept track of all our out-of-pocket expenses. I had to monitor her temperature closely. Another sheet kept those records orderly. When we visited doctors, many were quite impressed with my journal!
I realize that “Documenting Care at Home” is meant for long term care. My situation only required two months of extensive care, but without the guidance I received from this course, I would have been in a much more difficult situation. I learned that one never knows when they will be called on to provide medical care for a loved one! I sincerely thank Ms. Falzo for this invaluable guidance.

This man's experience is something that too many people experience these days within our healthcare system that has become so fractured.
This is why I created this course---to help people in this very kind of circumstance by providing indispensable tools to deal with these kinds of situations.

Hello Everyone,
I am excited to announce that I will be teaching classes of "Documenting Care at Home" from May-October in the church hall at Trinity Episcopal Church. 1336 First Avenue in Watervliet, NY 12189.
There will be 9 classes where I will teach the entire course in one session. There will also be a one-time Two-Part class in August for those who prefer to take the course with more time to learn.

If you know anyone who might benefit from this course, I urge you to encourage them to look into it as I truly believe that it has much to offer.
I will be teaching Part II of the course (Part I was 4/30/25) at the Adirondack Community College in Wilton/Saratoga this Wednesday (5/7/25).

Some exciting news!! On March 7th there will be an "International Women's Day" event at the Beltrone Living Center at 6 Winners Circle Albany, NY from 2:00 PM - 4:00 PM. I am posting the announcement as it appeared in "The Colonie Senior Connections" flyer. I will be manning a booth for my course "Documenting Care at Home". Hi, Everyone,

I am so excited to share with you the latest update for Documenting Care at Home. I attended 2 gatherings to be available to speak with people about the course and to hear about their experiences and their needs. The first gathering was at "The Elder Law Summit" held at the Queensbury Hotel in Glens Falls, NY on October 30th. Presented by The Conkling Center of Glens Falls. The Herzog Law Firm presented information on Estate Planning/Documents and Asset Protection, Medicaid/Nursing Home and Community, Aging in Place, VA Benefits, and Scams. There were vendors attending to inform people of different kinds of help available and I was one of the participating vendors. I met some wonderful people and engaged in some very interesting conversations, learning as well as sharing information. It was a very productive day.

On November 2nd, I again attended an event, The Watervliet Senior Marketplace held at the Watervliet Senior Citizen Center. There were vendors offering everything from The Pampered Chef to Homemade baked goods (which of course I HAD to investigate!!!)
Again, this was a very productive day, meeting some wonderful people exchanging ideas and hearing about their varying experiences and needs.

On December 6th, I will be attending The Cohoes Senior Marketplace to be held at the Cohoes Senior Citizen Center from 10:00-2:00. I understand this has been a very successful event in the past and is expected to do the same this year. I will have a table with lots of information and look forward to not only sharing what I have to offer but also to listen to what people have to share about what is happening with them. If you find yourself near 10 Cayuga Plaza in Cohoes (not far from The Cohoes Music Hall) stop by and say hello. I would love to see you and talk with you.

On November 17th, I will be teaching the Course at The Trinity Episcopal Church Hall 1336 1st Avenue in Watervliet. Registration will be from 1:00 -1:30 PM.
Class will begin at 1:30 PM and finish at approximately 4:30 PM.
Price of the course is $49.95. Credit Cards/Debit Cards only please. Please RSVP by phone at (518)727-0790 by November 12, 2024, as I need to make sure to have enough course packets for each participant. This will be the only live lecture until spring and because weather could be a factor the class will be held in one sitting. There will be breaks as needed to allow people to replenish themselves.

In the spring, I will be teaching the course at the Adirondack Community College in Wilton. The course will be taught in 2 Parts.
The dates are: -

Hi, Everyone,
It has taken me a long time to prepare this and get it up and running and there is still some work to be done.
For those who might not have heard or read about this course, "Documenting Care at Home", let me give a quick overview.
I developed this system of documenting care when I was taking care of my friend, Eliane and I have put it together into a course to teach people how to document the care being given in the home or for people to monitor their personal medical conditions that require constant management (such as diabetes).
It is comprised of pre-designed, ready to use documents. It is not an all or nothing system. One can pick and choose what documents and components work for them.

Anyone going to the website has free access to the 10 Page "Bibliography of Resources" and can make as many copies as desired and give them to friends.
No one wants to think about these things. I compare it to putting money aside for an emergency. You may never need to use this system and frankly, I hope you never do. However, the reality is that any one of us could be faced with a serious illness even if only temporary and there is rarely a warning.
I hope you will take time to look at the website. This is a system that can help so many people who are trying to manage their own medical issues or trying to care for someone in the home...a situation that has become extremely common place in our present-day healthcare system.
I welcome any comments or suggestions and if you know anyone who is in this kind of situation, please suggest that they at least take a look at the website and take advantage of the valuable information in the "Bibliography of Resources".
