Support Coordination Services of Utah

Support Coordination Services of Utah SCSU oversees and coordinates supports for individuals with developmental disabilities throughout the State of Utah.

What is Support Coordination?
• ASSESSING the needs, wishes, and desired outcomes of each individual

• DEVELOPING a Person Centered Support Plan (PCSP) with the assistance of the individual, their family, and those identified as part of the team

• LOCATING the supports and services identified in the PCSP so as to meet the needs of the individual and their family

• LINKING the consumer and family with service providers and other generic community resources

• COORDINATING the identified supports and services as requested by the individual and their family

• MONITORING supports and services to assure that the PCSP is implemented, that the PCSP continues to meet the needs and desires of the individual, and that the health and safety of the individual is assured

• MAINTAINING eligibility information and assisting individuals to apply for assistance for which they are eligible

• ADVOCATING for needed supports and services not readily available to the individual

On Tuesday September 13th the Utah Center for Assistive Technology (UCAT) will be hosting an open house at the Judy Ann Buffmire Building. The open house will run from 3pm to 6pm. Come join them to learn about the latest assistive technology devices, IPad, android and computer accessibility, Modified vehicles and so much more.
