Why I Offer In-Person, In-Office Wellness-Based Menopause Care Visits vs Insurance-Based Care in Peri/Menopause Hormone Therapy

Being Menopause Society certified and educated on advanced hormone optimization gives me a broad scope to understand what women need at this point in their lives . Long before the recent advent of online telehealth companies ( one of which, I briefly worked for…) I knew how to prescribe hormones to women ( and men). Experience counts. I have been prescribing HRT/MHT to women and men for over 13 years; even during those “dark days” under the shadow of misinformation propagated from the WHI study. I never believed hormones to be “bad,” quite the opposite and for the vast majority, beneficial.
My patients typically receive an in person, face to face 45–60 minute initial visit with me, that includes listening to YOU, hearing what you are telling me, then making a detailed, personalized risk assessment, education on benefits/risks of MHT, discussion of your unique options, lifestyle counseling, and shared decision-making. Your personal safety is always considered first and foremost. Often, from day one, you are started on a low-dose transdermal estradiol with appropriate progesterone; with close follow-up and dose titration based on symptom response and tolerability. I offer testosterone too. With dosing that may actually offer some benefit for women. Sadly, testosterone for women is still not FDA approved ; therefore, we have a good discussion on this subject when we meet if testosterone is something that interests you.
In a typical insurance-based model, the visit may be brief, limited by formulary restrictions, coding, and delays for potential prior authorizations. Patients may be prescribed a covered formulation without discussion of alternatives, or individualized goals, and your follow-up may be minimal and insufficient.
