ClearPath Hospice of Wichita is a locally owned hospice brought together by a group of dedicated hospice professionals who knew there was a better way to serve our community. The daily mission of ClearPath Hospice is straight-forward and direct; we will always provide individualized care and attention to our patients that meets their changing needs and exceeds their expectations. As an individual

entity, we do not have a corporate parent to tell us we cannot do the right thing. And we embrace faith as a guiding and divine force that wraps its love around our patients and our caregivers. We are here to serve the patients and their families who have put their trust and belief in our abilities. The team members who serve our patients at ClearPath Hospice answer the calling of serving those in need of palliative and end of life care; we knew there was a better way of doing it, and we’re proving it every day. Our Team Consists Of:

Medical Directors and APRNs. Our team of physicians and nurse practitioners are all dedicated hospice professionals whose skill and dedication to palliative care ensures that our patients are receiving world-class medical care. RN Nurse Case Managers, all of whom have extensive previous hospice experience, are highly skilled in providing symptom management and end-of-life care. Our nurses display the skill and heart to give their patients personalized care, support and love. Intervention Team, comprised of RNs and LPNs, who respond to on-call visits on nights and weekends, extra visits, and perform continuous care when necessary. This continuity of care for our patients serves a vital role in their security and peace of mind. Medical Social Workers (LMSW), who provide resources and counsel for families and patients alike. Their access to local, state and federal resources as well as coordination of providers brings together the different elements of each patient’s care. Chaplains, whose experiences in grief and the healing process are invaluable and provide comfort and spiritual support to the patient and their family. Bereavement and Volunteer Coordinator, who brings together volunteers to spend quality time with the patients and is there for family comfort and support. Hospice Aides, who provide the personal care for the patient and spending quality time with them on a personal level. We have been fortunate enough for these talented individuals to come to us; we have quickly become an employer of choice and do not have to recruit staff. The ClearPath Hospice Difference

The patient and their family decides what levels of participation they want – number of visits by the nurse, aide, social worker, chaplain, and others. We are here to serve. We have the most experienced team of senior care professionals in the market – two former Directors of Nursing and a LTC Administrator are part of our team. Highly seasoned, highly compassionate, and highly motivated staff who work here for the right reasons. The ability for you to direct care with an interdisciplinary team that works with you. Frequent feedback and communication, as we are all striving to be the very best. Quick response times, 24-hour access to staffing, equipment, and pharmacy. Specially trained on-call nurses who responds to the patient’s needs – no answering service. Advanced nursing skills, such as IV therapy, wound therapy, home infusions, and access of port catheters. We also have access to a 24/7 pharmacy who can compound meds for us. We are able to provide the families with bereavement care for 13 months post-death, providing them with a full range of support to ease them through the transition. Our volunteer program provides opportunities for community volunteers to work directly with our patients or in our office setting. The highly unique advantage that the team you are working with has the ability to make all decisions. We don’t have to ask permission to do the right thing.
