This is a 13 year old who sustained a soccer injury to her left knee. This sequence of pictures shows fixation of an osteochondral fracture of an adolescent’s knee using Biocompression screws to restore normal anatomy after injury. The defect shown is on the end of the femur on the outside of the knee. This injury is typically due to a sheering type force such as when the patella dislocates and chips off a piece of the cartilage. This fixation is to allow normal function to be restored and prevent future arthritis. @ Wilmington, North Carolina
An intraoperative Lachman Test is being performed in this video for assessment of an anterior cruciate ligament (ACL) tear. This test is a very sensitive diagnostic test that is used clinically to assess an ACL tear. The left knee shows a normal Lachman test. This is depicted with the knee having a clear stopping point. However, on the right knee the Lachman Test is grossly positive with no end point. Also, a positive pivot shift is shown in the right knee due to the instability created by the ACL tear.

This sequence of images shows a patellar tendon rupture with subsequent repair. This new technology using an Internal Brace made of suture rather than metal allows for early rehabilitation, decreases tension on the repair, and avoids the need for hardware removal after healing.

At CFSM, painful osteoarthritis of the knee associated with MRI confirmed subchondral edema consistent with insufficiency fractures, can be treated with SUBCHONDROPLASTY in select patients. This is a new technology to stabilize painful insufficiency fractures of the tibia and femur frequently associated with underlying osteoarthritis, using injectable calcium phosphate performed in combination with knee arthroscopy to address any other pathology.

AUTOGRAFT vs ALLOGRAFT: Decision making for ACL reconstruction
An autograft is one's own tissue harvested to reconstruct the torn ACL. An allograft is cadaveric tissue which is processed to limit bacteria viral exposure to the patient. This processing may weaken the tissue and lead to increased risk of ACL graft failure compared to autograft use, especially in the young athlete.
Our goal at CFSM is return to preinjury level of participation with decreased risk of graft failure!

At CFSM, we use cartilage implant procedures to prevent early arthritis, control pain and allow return to sports after cartilage injuries: This is a Cartiform procedure using an allograft bone and cartilage combination to replace a 2 cm cartilage injury in a young athlete. Cartiform grafts can also be used in cases of Osteochondritis Dissecans to graft cartilage defects.

OATS Procedure: Osteochondral Autograft Transfer Surgery
Using a cartilage plug from a nonarticular donor site to replace damage cartilage on a critical weight bearing site. This is done in a single stage procedure. At Cape Fear Sports Medicine, we use a variety of autologous and allograft cartilage procedures to PRESERVE the knee joint.

MENISCAL REPAIR = preservation of normal meniscal anatomy thru repair vs removal(menisectomy).Repairing the meniscus maintains the normal biomechanical shock absorbtion capability of the meniscus and may prevent early arthritis. At CFSM we have extensive experience with meniscal repair techniques with, and with out combined ACL injuries.
